Annual Turnover statements of the bidder in the three financial years viz., 2022-23, 2023-24 & 2024-25 for all bidders and last two preceding Financial years viz., 2023-24 & 2024-25 for MSMEs duly certified by the Charted Accountant (Annexure I)

GST- R 3B Challan for the month of March-2025 issued by the competent authorities of Commercial Tax Department

GST Registration Certificate

Copy of PAN CARD

Certificate of Incorporation issued by the Registrar of Companies (ROC) in case of a company/firm or Proprietorship Certificate in case of a proprietary firm in valid format

GMP or ISO or BIS Certificate in a valid format whichever is applicable

Valid SSI Certificate issued by Director of Commerce & Industries, Government of Karnataka in case bidder is an SSI unit in Karnataka is mandatory in e-portal to mark the concerned certificate. Failure to do so will not be considered

Valid MSME CERTIFICATE if applicable

Import licenseValid Import License issued by Competent Authority( for Imported Items only) and copy of authorization letter from the original manufacturer from the country of origin. Also furnish document regarding service and training centers located in India and list of service engineers based in Karnataka

Brochure /catalogue for items in Section IV of this tender document

Details of manufacturing Unit and production capacity duly signed by concerned authorized signatory (ANNEXURE II)

List of items quoted by the bidder (Annexure-IV)

Undertaking to print logograms as per the format duly signed by the authorized signatory as per (Annexure-V)

Tender Offer cum Declaration Form (Annexure VI)

Form of Authorization for Authorized signatory (to take all the decisions regarding this tender to be uploaded in the Company Letterhead) (Annexure VII)

Notarized affidavit declaration by the Tenderer regarding Non-Debarrment of the firm . The firm must not have been debarred by any departments/corporations /statutory bodies of any states of India at the time of uploading the tender as per (Annexure VIII)
