Procurement of ECG Machine and kits for Heart related disease for 12 Hospital of HDMC

A list giving full particulars, including available sources and current prices, of spare parts, special tools, etc., necessary for the proper and continuing functioning of the goods for a period of two years, following commencement of the use of the goods by the Purchaser.

A) Bidder as a manufacturer should have supplied min 80% (However it will be 40% with respective valid micro & small enterprises) of tender quantity. Supplier should enclose supply order/purchase order along with Performance certificate of supplied 10 nos ECG Machine to any ULB/other state or central Govt Departments or any Govt. Institution in any one of the last 3 years i.e 2022-23 to 2024-25 B) The tenderer, as authorized representative of the manufacturer meeting the above requirement, shall have supplied, in his name satisfactorily at least 50% of tendered quantity. Supplier should enclose work order/supply order/purchase order along with Performance certificate of supplied 06 nos ECG machine to any ULB/other state or central Govt Departments or any Govt. Institution in any one of the last 3 years i.e., 2022-23 to 2024-25

Each tenderer should further demonstrate Line of credit/Bankers Certificate facilities of not less than Rs 4.50 lakhs with name of the work and Indent Number from any Nationalized Bank (as per the Section-XV)

Achieved in any two years of the last three financial years an annual financial turnover of Rs. 60.00 Lakhs. Balance Sheet/ Profit & Lost Statement/ Income Tax Returns Statements for last 3 years i.e 2022-23 to 2024-25 Certified by Reg. Chartered Accountant with UDIN No

Bidder should enclose valid manufacturer certificate like MSME/NSIC or Manufacturer Certificate issued by DIC or Any other valid competent authority For Authorised Dealers Dealership certificates issued by MAF (Manufacturers Authorisation Form) as per section XIII mentioned in the tender document

MSME or NSIC ISI or ISO or BIS or CE Certificate for applicable items

The bidder is an authorized dealer than he shall upload authorization letter issued by manufacturer (OEM) (as per authorization form given in Section XIII).

Tenderer should produce the samples ECG machine before submission of the tender and get the acknowledgement from Executive Engineer (Env) Hubbali-Dharwad City Corporation (HDMC), Hubballi office and upload the same in KPP Portal.
