Letter of authority issued by the manufacturer authorizing the tenderer to quote his product for the current tender in original

List of Similar supplies made in the last three years, Users Certificate as per format Section-V and Supply order copies.

Copy of Manufacture license for the equipment, if tenderer is a Manufacturer else Authorized Agency certificate from Principal manufacturer in original in format Section IV if tenderer is an authorized agent and Also submit the Manufacturer License.

Self declaration stating the firm is not black listed with any Government /Quasi Government Organizations and will not supply Refurbished equipments
