Tender Offer Form duly filled and signed by the supplier as per the format enclosed in Annexure-1

Compliance of technical specifications complete with clarifications for deviations

Relevant detailed brochures/literatures of their company pertaining to equipment complete with make, model, country of origin, year of manufacture, end of manufacture, end of service of equipment.

Tender Declaration Form duly filled and signed as per the format enclosed in Annexue-2

Attested copy of the latest Manufacturing License

If representative or dealer or distributor is submitting tender, he must produce copy of such authorization Certificate from the manufacturer (Annexure 5) and no more than one person can make tender on behalf of same manufacturing firm or company for same material.

Complete details of the Manufacturing unit (Annexure-7) should be uploaded

Tenderer should compulsorily upload audited balance sheet for 3 years i.e. 2022-23, 2023-24 & 2024-25 duly certified by competent auditors

Warranty period or guarantee period particulars should be uploaded

Tenderer should upload the copy of the PAN Card

Quality Certificates for the equipment

User Certificate, Annexure-9

Service Centers in Karnataka & Trained Technical Staff available

Annual Turnover Statement for the last 3 years i.e 2022-23, 2023-24 & 2024-25should be compulsorily certified by competent auditors and uploaded

Sales tax/VAT/GST clearance certificate of the 3 years 2022-23, 2023-24 & 2024-25 from competent authority should be uploaded
