Proforma statement of the orders up to at least 80% (i.e. 4,00,000 Nos) for the primary manufacturer and up to at least 30% (i.e. 1,50,000 Nos) for the authorized representative/agent of a manufacturer of the scheduled requirement of the tender for the supply of D Pharma theory answer Scripts / OMR Answer booklets with QR code on all the pages containing 32 pages of A4 size in any one of last three years (i.e. 2021-22, 2022-23 and 2023-24) in Section - XII and the Performance Statement / Satisfactory Certificates of the same should be submitted.

Self-Declaration stating that the tenderer have not been black listed by any government department or Quasi Govt. Organizations or Public Undertaking Sectors and will not supply Refurbished materials and is not facing any judicial enquiry

Earnest Money Deposit (EMD) should be paid and receipt (proof) of the same should be submitted as per Section - IX.

The bidder has to upload the copy of the PAN Card.

Self-Attested Valid Registration Certificate of GST.

Information in Section - XIV Proforma for items / goods and quality control employed by the manufacturer to be submitted.

If the tenderer is the primary manufacturer, the declaration of primary manufacturer in Section - XV as applicable to be submitted. If the tenderer is other than the primary manufacturer, the Authorization letter from the Primary manufacturer should be in the format provided as per Section - XIII to be submitted.

Technical Specifications should be filled (compulsorily with model no, part no and if any other required details), signed, and submitted as per Section - VI.

Copy of valid Firm registration / Company incorporation certificate.
