Average Annual Turnover of the vendor for the last three completed financial years prior to date of opening of tender should be equal to or more than three times the estimated annual value of purchase (Estimated annual value of purchase is Rs 4,05,000/- and required financial turnover is Rs 12,15,000/-). Vendor should submit profits in at least two out of the three last three financial years. Balance sheet Profit - loss account certified by CA should be submitted.

Performance certificate / Experience certificate for of supply of drugs/surgical items to Government/Private/Corporate Hospitals should be submitted.

Please enter the percentage of local content in the material being offered. Please enter 0 for fully imported items, and 100 for fully indigenous items. The definition and calculation of local content shall be in accordance with the Make in India policy as incorporated in the tender conditions.

The tenderer has to submit their details as indicated in Annexure II.

Pharmacy Council Registration details of Full Time Pharmacist employee (In case of retailer). For wholesales it is required to submit the educational qualification of CP.

Drug retailer/wholesaler must hold Valid Drug retailer/wholesaler must hold Valid Drug License as on date of submission of bid in specified forms issued by Drug Control Authority of the state under the provision of Drugs and Cosmetics Act 1940. as on date of submission of bid in specified forms issued by Drug Control Authority of the state. Drug retailer/wholesaler/ should be in existence continuously for at least five years. (Copy of license held for last five years to be submitted).

Non-Conviction Certificate issued by the Drug Control Department duly indicating these tender details to be submitted. Tenderers can also submit the copy of acknowledgement of the application at the time of participation. However, the empanelment is only on production of the Non conviction certificate.

Balance sheet Profit - loss account certified by CA at least two out of the three last three financial years should be submitted.

Copy of valid GST Registration certificate.

Copy of PAN allotted to the firm.

Performance certificate / Experience certificate for of supply of drugs/surgical items to Government/Private/Corporate Hospitals should be submitted.

The tenderer needs to submit of undertaking certifying the truthfulness of all the documents submitted/uploaded as on Annexure I.
