RequiredAnnexure V (A) & (B) - Full details of owned/hired vehicles with Color Scanned Originals of RC Books, TAX receipts, latest Form 24 B (Registered abstract issued after the tender notice date), Fitness certificate, vehicle insurance and valid permits declared in the tender shall be uploaded. A color Scanned Original hire agreement valid for at least 1 year for hired vehicles.

OptionalIn case of Partnership firms, the firms which have a minimum of (1) year of existence prior to the date of tender notification are only eligible and Original colour firm registration certificate shall be uploaded.

OptionalIn case of a limited Company, copy of the Memorandum and Articles of Association, and copy of the Original colour firm/company registration certificate shall be uploaded.

RequiredAnnexure- VI - Prescribed Financial undertaking Form.

RequiredAnnexure VII - Affidavit for Lorry Owner Associations. (if applicable)

RequiredScanned Copies of Aadhaar, PAN & GSTIN(if applicable).
