SCHEDULE 'A' PART I . Repairs/ maintenance of Electronic Nurse Call System (in ward) / Electronic patient call system (in OPD,MI ROOM & OT) consisting of the following items including replacement of minor unserviceable parts such as supply indicating on/off switch, input/out put connectors board, power indicating PCBs, counter attendance PCBs, Flexible cord/ wiring cables, pins, clips etc. required to maintain the serviciability of the system other than those mentioned in shedule A Part II complete all as specified and directed Make : CR MEDI SYSTEMS ( Electronic Nurse Call System In Wards) (a) Central Display Console for : (i) Capacity upto 6 beds : 03 Nos. (ii) Capacity from 07-12 beds : 03 Nos (ii) Capacity from 13-18 beds : 02 Nos. (iii) Capacity from 19-24 beds : 07 Nos. (iv) Capacity from 25-30 beds : 05 Nos. (v) Capacity upto 54 beds : 01 No. (b) Bed Panel Module : 441 Nos. (c) Hand Held Module : 441 Nos. Make : CR MEDI SYSTEMS (Patient Call System in OPD, ,MRI CT Scan Dept.,MI Room & OT)

(a) One Central Display Console in waiting area with two door display units near door and two room control units : 12 Nos (b) One Central Display Console in waiting area with three door display units near door and three room control units : 01 No (c ) One Central Display Console in waiting area with four door display units near door and four room control units : 01 No. Note:-1.The repair and maintenance of the Electronic Nurse Call System and Electronic Patient Call System shall be carried out only by the OEM, M/s CR MEDI SYSTEMS, or their duly authorized service representative. The contractor shall produce valid authorization from the OEM before commencement of work whenever demanded by the Engineer-in-Charge. 2.All replacement parts shall be genuine OEM-approved components. 3.The contractor may visit the site for inspection with service engineer of OEM/Manufacturer of the system before quoting his rate. 4.The contractor shall maintain proper record of repair/maint in a binded register/logbook.The register shall clearly record the details of each service visit, including : (i)Date and time of service visit.(ii)Location/Ward/Department. (iii)Nature of complaint received.(iv)Fault diagnosed. (v)Repair/maintenance carried out. (vi)Details and quantity of spare parts/components replaced. (vii)Functional testing carried out after repair. (viii)Status of the equipment/system after completion of repair. (ix)Name and signature of the service engineer.The maintenance record/register shall be duly authenticated by the concerned JE E/M.The contractor shall put the maint record register to Engineer-in-charge for inspection when asked for.
