RequiredPerformance report of works as per FORM- referred to in FORM-C NOTE In case the performance report is issued for private work, the bidder shall also upload scanned copy of 26 AS along with BOQ paid in final bill.

RequiredPermanent Account Number (PAN) as issued by the Income Tax Department

RequiredCopy of valid elect. License for execution of work in Bihar State or an undertaking stating that I We will obtain valid electrical license at the time of execution of electrical work.

RequiredIf the bidder is a firm in partnership, the bid shall be signed by all the partners of the firm above their full type written names and current address, or, alternatively, by a partner holding power of attorney for the firm. In the later case, a certified copy of the power of attorney should accompany the bid. In both cases a certified copy of the partnership deed and current address of all the partners of the firm should accompany the bid.

RequiredIf the bidder is a limited company or a corporation, the bid shall be signed by a duly authorized person holding power of attorney for signing the bid accompanied by a copy of the power of attorney. The bidder should also furnish a copy of the Memorandum of Articles of Association duly attested by a Public Notary.

RequiredBankers Certificate as per FORM- B or Net worth certificate as per FORM- B-1 for non CPWD Bidders .
