Cost per test of HbA1c investigation Test including reagent(s), controls, calibrator(s), wash solution(s) and all other consumables required

Cost of External Quality Assessment (EQAS) Programme

Sample Vials / Sample Cups

Printer Paper / Thermal Paper / Reporting Consumables

Cost per test of any other investigation available on the quoted model including reagent(s), controls, calibrator(s), wash solution(s) and all other consumables required

Note:- 1. The Basic Rate per unit can be quoted in INR Only. 2. Total Cost per Unit quoted in 1.01 to 1.07will be taken for evaluation of L-1 Bidder (Including Basic Rate+ GST+ CMC).
