Notes for serial item No 01 to 14:- (i) The details of maintenance to be carried out during daily /fortnightly /monthly /quarterly / half yearly /yearly should be all as per details attached in Appendix 'A' Proforma I to VI. (Particular specifications of contract). Log sheet of the maintenance will be submitted by the contractor on monthly basis duly signed by service engineer and contractor. Format of logsheet will be approved by Engr-in-charge. (ii) The rate quoted is inclusive of the cost of all kind of checking and for earth resistance test. (iii) The rate quoted is inclusive of repairs /replacing of earthing system as required on ground. (iv) The rate quoted is inclusive of the cost of components generally changed due to wear and tear and not considered under Sch A part III, such as parts of electronic control panel's power contactor, break shoe liner, cables for inter connections, springs, bearings, bushing, gibs, fuses, nuts and bolts etc. (v) The visit of executives/ experts be intimated well in advance so that periodical maintenance can be supervised, schedule of periodical maintenance (daily, fortnightly, monthly, quarterly, half yearly and annually) be submitted well in advance for approval by Engr-in-charge. (vi) The AMC should cover any/ all parts of lifts if its functions found defective the same should be repaired or replaced by the OEM as per the timeline covered under norms. (vii) The contractor has to carry out the AMC of the lifts through OEM only. (viii) All maintenance should be carried out in presence of concerned JE E&M and OEM Authorised Lift engineer maintenance slip will be submitted accordingly. (ix) The rate quoted is inclusive of annual inspection of lift by PWD authorised lift inspector and the report should be submmited to GE office accordingly. (x) Contractor has to upload the MOU from OEM after award of contract.

Providing Lift Technician for passenger lifts of capacity 8 passengers (544 Kgs) and 13 passengers (884 Kgs) - 06 Nos of Make : M/s Monarch Enterprises, installed at Bramhaputra, Beas & Betwa buildings from 0600 hours to 1400 hours and 1400 hours to 2200 hours in two shifts of 08 hours each per day, including Sunday and holidays all as per latest applicable lift rules complete all as specified and as directed by Engr-in-charge.

Providing Lift Technician for passenger lifts of capacity 8 passengers (544 Kgs) and 13 passengers (884 Kgs) - 07 Nos of Make : M/s TK Elevator, installed at Sangam, Zhelam and Yamuna buildings at 140 DSC Married Accn from 0600 hours to 1400 hours and 1400 hours to 2200 hours in two shifts of 08 hours each per day, including Sunday and holidays all as per latest applicable lift rules complete all as specified and as directed by Engr-in-charge.

Providing Lift Technician for passenger lifts any capacity and any Nos of Make : M/s Monarch Enterprises, installed at Archana, Alka and Aradhana buildings from 2200 hours to 0600 in one shift of 08 hours each per day, including Sunday and holidays all as per latest applicable lift rules complete all as specified and as directed by Engr-in-charge.

Providing Lift Technician for passenger lifts any capacity and any Nos of Make : M/s Monarch Enterprises, installed at Bramhaputra, Beas & Betwa buildings from 2200 hours to 0600 in one shift of 08 hours each per day, including Sunday and holidays all as per latest applicable lift rules complete all as specified and as directed by Engr-in-charge.

Providing Lift Technician for passenger lifts any capacity and any Nos of Make : M/s TK Elevator, installed at Sangam, Zhelam and Yamuna buildings at 140 DSC Married Accn from 2200 hours to 0600 in one shift of 08 hours each per day, including Sunday and holidays all as per latest applicable lift rules complete all as specified and as directed by Engr-in-charge.

Note for Serial item No 16 to 21 :- (i) The lift Technician / rescuer (operator) shall be Matriculate with sufficient experience (not less than 05 years). The lift rescuer (operator) should be capable to rectify both mechanical as well as electrical defects for smooth functioning of lifts. It is responsibility of contractor to provide a reliever to the rescuer (operator). So that lift rescuer (operator) is available all time during operation period for uninterrupted functioning of lift. (ii) Man power list with Aadhar linked bank account number, qualification certificate will be submitted prior to commencement of Rescue Operation for necessary sanction from GE. (iii) The contractor/ service providing agency shall be wholly and exclusively responsible for payment of wages of persons engaged by it in compliance with all the statutory obligations under all related legislations as applicable to it from time to time including Minimum Wages Act, ESI Act and the Deptt. MES shall not incur any liability for any expenditure whatsoever on the persons employed by the contractor/ Agency on account of any obligation. The contractor/ service providing agency will be required to provide particulars of Group Insurance of its Employees engaged by him to the GE before processing of RAR payment. (iv) The contractor/ service providing agency will be required to provide two sets of uniform to all operators with name and firm tag.

SCHDULE 'A' PART-III (ESSENTIAL REPAIRS) Material and labour for rewinding of double speed electric motor 3 phase, 50 Hz, 415 Volts comprising of following capacity complete all as specified and as directed by Engr-in-charge. (i) Disconnecting and removing electric motor from installed position loading and unloading transporting to nearest workshop. (ii) Dismantling existing burnt winding from motor, cleaning of motor and rewinding complete with super enamelled copper winding wire of existing gauge including insulation material as required. (iii) Bringing back electric motor duly rewinding including loading and unloading transporting from workshop to site of work including electric connection, commissioning and testing in actual working condition for 8 hours continuously to check RPM, Ampere, load and overall performance. (iv) Replacement of ball bearing - 02 Nos for each motor as per existing size and make RPM.

Material and labour for replacement of brake liner for 10 HP motor complete all as specified and as directed by Engr-in-charge.

Material and labour for replacement of coupling bush rubber for 15 HP motor complete all as specified and as directed by Engr-in-charge.

Disconnecting carefully and transporting to authorised workshop including loading and unloading and repair to Variable Voltage Variable Frequency drive (VVVF drive) by authorised mechanic of 'Monarch' make of lift including replacement of damaged parts /chips in the drive and again transporting back to site of work reconnecting testing complete to the entire satisfaction of Engineer-in-charge complete all as specified and as directed by Engr-in-charge.

Disconnecting and taking out unserviceable COP assembly and supply, fixing and testing new car operating panel assembly complete with illuminated push button for floor position, alarm on stop, door close, door open, cabin fan, cabin light, etc., in and including direction indicator upto 7 segment display with CPI card and stainless steel face plate, duly connecting with existing control and electric system etc., to serve G+06 floors for 'Monarch' make lifts complete all as specified and as directed by Engr-in-charge.

Removing and replacing the existing unserviceable LED display card and push button in call box complete all as specified and as directed by Engr-in-charge.

Supply and fix in repairs existing unserviceable infra red door protection edge/ sensor (Lambada) with latest new infra red door protection edge for full height of car door edge for 08/13 passenger or goods lifts of 'Monarch/TK' make lift complete all as specified and as directed by Engr-in-charge.

Material and labour in replacement for car door motor with movement wheel suitable for 'Monarch' make lift complete all as specified and as directed by Engr-in-charge.
