Authorized sales agencies/organizations should be financially sound. Authorized manufacturing industrial organizations should mandatorily submit copies of orders regarding the supply of motorized two-wheelers for the disabled for 50% i.e. Rs. 6.00 lakhs to any government/semi-government departments in the years 2023-24, 2024-25 and 2025-26 and satisfactory certified documents regarding the supply from the respective departments.

A certificate stating that the tenderer is an authorized dealer of the original equipment manufacturer/supplier should be attached. (Must be valid)

Registration Certificate of the organization registered with the competent authority. (Tenderers should attach a copy of the certificate of permission (IoCchichita Munhodiies) obtained from OSOE/UOMO and Mahanagara Palike/Nagar Sabha/Municipality/Pattana Panchayat)

Attaching the Certificate of Incorporation (Motive Certificate)

The bidders submitting tender through e-procurement shall attach a document stating that they have a certificate of accreditation from the Transport Department/Principal, Polytechnic for the installation of a diesel engine unit and modification of the engine to the engine of a motorized two-wheeler (Uchidi Iess).

Attach a letter of consent stating that the engine, transmission, and engine oil will be installed as per the design of the two-wheeler manufactured by the supplier or manufacturer.

The tenderers should attach a copy of the Pdg. Chitija Ioss Hchitachitichchie shee for the years 2023-24, 2024-25 and 2025-26. A copy attested by the A.M. for each year should be attached.

A letter from the organization agreeing to deliver the motorized two-wheelers to the location specified by the department as per the design specified by the department must be attached.

The bidder should be an Original Equipment Manufacturer (OEM) authorized dealer of Original Equipment Manufacturer (OEM) of 125cc and above high-performance motorized two-wheelers. And should have a sales and service network in Karnataka. And should be involved in manufacturing of retrofit kits themselves. The bidder should have a contract agreement with other authorized retrofitters for retrofitting of additional two-wheeler attachment in such dual use as certified by the Department of Transport. Proof thereof should be submitted along with the tender documentation.

Attach a copy of the letter detailing the district where the vehicle service center is located in Gadag district. (Sample is attached with a telephone number.)

Submit a self-declaration letter certifying that the tenderer/organization is not blacklisted by any department, along with a stamp duty of Rs. 100/-, attested by a notary.

Income Tax (IT) Payment Certificate for the financial year 2023-24, 2024-25 and 2025-26

A copy of the audit report of the institution for the years 2023-24, 2024-25 and 2025-26 should be attached (Financial Statements)

Attach GST registration certificate.

The organization must mandatorily attach the USKHI-9 audit report for the last year 2023-24, 2024-25 and 2025-26 along with the certificate obtained from the Honorable Commissioner, Commerce and Taxation Department. (USKHI-9 NUMJIN KHETHIODINAS)

The tenderer shall obtain a copy of the GST Clearance Certificate for the years 2023-24, 2024-25 and 2025-26 (GST Clearance Document) from the Honble Commissioner, Commerce and Taxation Department and attach it. (Ukhi AttacheChidichittichie Aedinajichichichinae) (3Ah, 3Aaa) The document shall be attached.

The Income Tax Return for the years 2023-24, 2024-25 and 2025-26 should be certified and submitted by the Honorable Commissioner, Department of Commerce and Taxation.

Attach a copy of PAN card.

Submitting details to the OEI/UEI Ombud regarding payment of EMD amount.
