Day to day Operation/ Bed-cum passenger lifts by providing 2 lift operator per shift. Lift Operator 3 (Threet) shift throughout the year. For 3 shift per Day For Abinash Dutta Maternity Home, Phoolbagan Semiskilled Labour

The agnecy will be bound to produce ESI,PF challan when asked for by the EIC i)Cleaning / House Keeping Lift Machine Room ,Lift well , & Lift car to be done by the concern agency operating the lift . ii)The lift operator/attendant should have a sound knowledge with valid lift operator license issued by the Licensing Board, Govt. of West Bengal. iii)The operator/attendant should be obliging dutiful and polite to all while on duty. In case any irregularities of duty or complain against the operator/attendant if noticed, would be withdrawn forthwith and shall be replaced by new operator.

iv)The report is to be submitted periodically about the working of lift incl. all safety devices and Governor to the concerned JE/AE. v)Agency must have to produce the list of the staff mentioning their Name, contact Mobile No.in writing with proper photo identity and address proof & their Valid Lift Operating License issued by D.O.E., Govt of West Bengal along with photo copy to the concerned JE & AE before commencing the work. This should be treated as mandatory. vi)The agency will have to provide proper dress for his staff and color of the dress will be decided by the EIC. No staff will be allowed for performing duty without proper dress.

vii)The agency will issue a photo Identity Card, duly signed by the agency and countersigned by the concerned JE / AE to all the operation Staffs deployed including substitutes. They should always carry the said Photo Identity Card while on duty. The agency should also submit one copy of the said Identity Cards to the JE viii)The agency shall have to prepare a Duty Roster of the operation Staff at the beginning of every month, or as directed from time to time. A copy of the said Duty Roster, duly signed by all staffs, should be submitted to the concerned JE.

ix)The agency must have separate contact telephone numbers with Mobile Nos. for his office and Supervisor deputed at worksite so that the agency may be communicated in case of any urgency and the same should be intimated to the E.I.C., A.E. & J.E concerned, in writing. The telephone call should be responded without delay. x)Rescue operation is in the scope of agency in case of exigency arises out of lifts getting stopped in between landing floors with passengers inside cage. xi)The lift enclosure , Machine Room to be kept neat & clean regularly by the operator.

xii)The operational work of the lift is to be supervised directly under the supervision of technical staff of the contractors under the direct control of JE/AE concerned. The contractor shall be responsible for any damage to the lift due to mishandling, loss will be recovered from the contractors bill as per direction of EIC. xiii)The Log Book recording regarding functioning, checking of ARD of lift weekly and breakdown of the lift is to be maintained at site by the contractor under supervisions of JE concerned. If any kind of trouble arises, it should be brought to the notice of JE, AE concerned immediately. The JE/AE would put their signature on log book making note on performance.

xiv)In case of any absence of operator, alternative operator immidiately to be placed. In case of absence prorate deduction will be made from the contractors bill and the contract may be terminated if it exceeds 3 (three) days. In this respect Executive Engineer’s decision will be final. xv)This department would not be held responsibility for any accident of the liftman on duty for any reasons. The contractor or their lift operators on duty would not claim any compensation in any shape from this department for such accident.

xvi)The Executive Engineer,North Kolkata Health Electrical Division, P.W.Dte. would have sole authority to cancel the contract without assigning any reason with a notice of one month only. xvii)Any conditional quotation offered would not be accepted. xviii)Any quotation offered with price variation clause would not be accepted. xix)The payment will be made quarterly on submission of bill in triplicate by the agency with satisfactory report regarding performance by the concerned JE/AE. Copy of log book has to be enclosed with the bill.

xx)Documentary evidence in support of payment of minimum wages as per Labour Act to be produced to the Principal Employer i.e. Executive Engineer,North Kolkata Health Electrical Division, P.W.Dte. in-charge of works if any discripency or issue came under his notice . xxi)As per direction of EIC Lift Operator may be posted at any lift under R.G Kar Medical College Electrical Sub-Division, P.W.Dte. for on going and upcoming Lift if any.

xxii)The agency has to provide one GPS activated smart phone dedicated for call center. The cost of the handset and running cost i.e. phone bills have to be borne by the agency. No claim of payment against any telephone bill will be entertained by this department.The 24 hour x 7 days Available Phone number of Supervisor/Owner to be pasted on lift wall .xxiii)The supervisor/operator must inform JE/AE any Lift related problem at the earliest .

xxvi)The Lift Operator / Attendant Personnel, deputed by the agency preferably should be a permanent resident of West Bengal with valid Voter Identity/ Adhaar Card and should possess valid Lift Operator License, as mentioned below, issued by Directorate of Electricity, Govt. of West Bengal xxvii)Semi-Skill (Lift operator/attendant) =Lift Operating License issued by D.O.E., Govt of West Bengal

xxvi)The Lift Operator / Attendant Personnel, deputed by the agency preferably should be a permanent resident of West Bengal with valid Voter Identity/ Adhaar Card and should possess valid Lift Operator License, as mentioned below, issued by Directorate of Electricity, Govt. of West Bengal xxvii)Semi-Skill (Lift operator/attendant) =Lift Operating License issued by D.O.E., Govt of West Benga

xxviii)If The concern agency maintain or pay the minimum wages as per state government rule/Law as stand the 1 st january of the Acceptence reciving year . xxix)The Deduction pershift=(1/30)*(Rate/Shift/Month) should be deducted as penalty from bill amount in case any absent in per shift lift opration xxx) The payment to be done to the agency against bill depends upon availability of fund xxxi) The Log Book recording regarding functioning,checkingof ARD of lift weekly and breakdown of the lift is to be maintained at site by the contract or under supervision of JE concerned. If any kind of troublearises,it should be brought to the notice of JE,AE concernedimmediately.The JE/AEwouldputtheirsignatureonlogbookmakingnote on performance
