RequiredCertificates of Work Experience (Form C)/Projects under execution (Form C I)

RequiredPerformance report of works (Form D) Form D 1 need not be filled by the bidder. Marks will be assigned by the designated committee for quality of works at site based on the inspection of chosen works after opening of Technical bid.

RequiredAffidavit for non execution of works on back to back basis. (FORM J)

RequiredGST registration Certificate if already obtained by the bidder. (Annexure III) If the bidder has not obtained GST registration as applicable, then he shall scan and upload following undertaking along with bid documents.

RequiredCopy of Enlistment Order for CPWD Contractors who are enlisted in appropriate Class of Building and Road/ Composite Category

RequiredUploading of valid electrical license of eligible class Or Uploading of undertaking for obtaining valid electrical license at the time of execution of electrical work or association of Contractor having valid electrical license of eligible class.

RequiredDeclaration about Site inspection.

RequiredSigned copy of Letter of Transmittal.(FORM T 1)

RequiredBidder s Bidding Capacity in FORM C 3 certified by Chartered Accountant.

RequiredForm D 2/ MoU with associated agency for prescribed technology (As per annexure IV).

RequiredPAN card and any other document as specified in the NIT.

RequiredERP training certificate issued through LMS of ERP taken by himself or his authorized representative. Training will be valid only when participants are registered with LMS (Learning Management System).

RequiredCertificate of Financial Turnover from Chartered Accountant (Form A).

RequiredBanker s Certificate from a Commercial Bank (Form B) or Net worth Certificate (Form B1)

RequiredProforma of Affidavit for Non-Black Listing should be furnished on Rs.100/- non-judicial stamp paper attested by Notary. (Form F)
