For proceding three financial year 2022-23, 2023-24 and 2024-25 of the Manufacturer and if the Tenderer is an Authorized Agent /Distributor should upload the copy of Manufacturers Annual Turnover Statement of 90.00 lakhs along with his Annual Turnover certified by Commercial Tax Officer/ Charted Accountant as per Annexure-IV is to be uploaded

The tenderer should upload the Balance Sheet and Profit & Loss account and Income Tax Returns for preceding three financial years year 2022-23, 2023-24 and 2024-25 of the Tenderer certified by Chartered Accountant

Manufacturing license or the Distributor / Authorized Agent License from the manufacturer The bidder must have the valid Manufacturer License or the Distributor/ Authorized Agent License from the manufacturer for the quoted items

The tender document shall have seal and signature on all each pages including the Annexure by the authorized signatory (Vide section I 1(a))

Supply of Liquid Medical Oxygen and other Medical gases to various Government /Quasi Government/Autonomous Institutions with quantities/Performance Satisfactory certificate and work done certificate of Rs.30.00 lakhs during the financial year 2022-23, 2023-24 and 2024-25 (Do NOt Upload Supply Order)

Copy of the Self declaration in Rs. 100 /- bond paper should be taken in the name of District Surgeon, District Hospital, Haveri ie The firm is not in Black listed. Bond Should be taken after tender call dated and to be notarized

Submit the tender terms and condition copy with seal and signature

33% Bank Solvency certificate should be produced by the Nationalized banks

Non-Refundable DD of Rs. 5000 to be drawn in favor of The Director, HIMS, Haveri and to be uploaded in the kppp portal. The original DD must be produced at the the time of document verification
