The undertaking in the letter head of manufacturer as well as authorised dealer shall be uploaded stating The NABL accredited laboratory name, address, for the purpose of this tender means a third-party independent testing laboratory which shall not be a part of the manufacturing unit or not owned by the manufacturer/ bidder. Only for IV fluids.

The undertaking for the below shall be uploaded with details of NABL Lab. NABL accredited lab - the testing laboratories duly recognized by the national accreditation board for testing and calibration laboratories (a constituent board of quality council of india) for the technical competence for performing the specific tests with reference to international Standards. such laboratories should have NABL accreditation in relevant disciplines separately to perform quality tests for the tested category of items and shall be duly approved by the D and C Act in case of drugs. the NABL accredited laboratory should furnish certificate of analysis as per form 39 of d & c rules- Only for IV fluids.

If the bidder is manufacturer should hold valid WHO-GMP certificate (who-good manufacturing practices) in respect of the production units and the products quoted. If the offered products are manufactured from more than one unit, all the units shall be WHO-GMP certified should be uploaded. or If the bidder is authorized dealer/ distributor than bidder should upload the manufacturers valid WHO-GMP certificate (who-good manufacturing practices) in respect of the production units and the products quoted. If the offered products are manufactured from more than one unit all the units shall be WHO-GMP certified should be uploaded.

The MSMEs registered within the state of Karnataka shall hold valid cGMP certificate issued by the licensing authority for the products manufactured within the state Karnataka.
