Minimum Average Annual Turn Over i) If Bidder is a Authorized Agent/Distributor: Bidder Should have minimum average annual turnover of Tender value per annum for the Financial Years 2022-23, 2023-24, 2024-25 certified by Charted accountant as per Annuxer-II. And Authorized Agent/Distributor also has to furnish minimum average annual turnover of 10 crores of Respective Manufacturers for the Financial Year 2022-23, 2023-24, 2024-25 certified by Charted accountant as per Annuxer-II. ii) If Bidder is a Manufacturer: Bidder Should have minimum average annual turnover of 10 Crores for the Financial Year 2022-23, 2023-24, 2024-25. Certified by Charted accountant as per Annuxer-II.

Copies of Balance Sheet and Profit and Loss Account for preceding three Financial years i.e. for years 2022-23, 2023-24, 2024-25 certified by Charted Accountant / Auditor, shall be uploaded otherwise will be considered as Non-Responsive

The tenderer should upload the Income Tax Returns for three Financial years i.e. for years 2022-23, 2023-24, 2024-25 of the Tenderer.

List of Drugs/items quoted by the tenderer to be furnished separately as per Annex IVA.

a) Copy of the Manufacturing license issued under the Drugs & Cosmetics Act 1940 and rules, duly renewed up to date along with list of products permitted. (Items to be quoted) should be uploaded, otherwise will be considered as Non-Responsive.b) In case of Manufacturing License applied for renewal, it is insisted to upload the validity certificate from the licensing authority in respect of their license that it continues to be valid during the period of tender process i.e after submission for the renewal, till the time the license is actually been renewed, shall be uploaded, otherwise will be considered as Non-Responsive

Bidder shall submit Valid No Conviction Certificate In Favor of The DIRECTOR KMCRI HUBBALLI issued not older than six months from the tender notification date issued by the concerned Drugs Control Authorities as per Annexure-VII / Online Certificate Copy shall be uploaded otherwise will be considered as Non-Responsive

Performance Certificate from Government/Autonomous Institutions/Private Hospital having 1000 or more bed strength in Karnataka or outside Karnataka for the last two years i.e for the years 2024-25 & 2025-26 as per the Annexure- IX is to be uploaded. otherwise will be considered as Non-Responsive

The form of Authorization, referred in Annexure XVI, in original, shall be uploadedotherwise the tender will considered as Non Responsive

Tender Offer Form with an undertaking to abide by the terms and conditions of the tender in the format as per Annexure XIII- part. I shall be uploaded, otherwise will be considered as Non-Responsive

Declaration from the Tenderers in the format as per Annexure XIII-part. II shall be uploaded, otherwise will be considered as Non-Responsive.

Letter of Undertaking to print logogram as per the format. Annexure X

The tender document shall have Seal and Signature on all pages including the Annexures by the authorized signatory

Distributor License duly renewed upto date/license validity certificate from Drug Controller for Drugs & Authority Concerned for Non Drugs.

Valid SSI certificate issued by Director of Industries Commerce, Government of Karnataka shall be uploaded, by SSI/Micro and Small Enterprises units of Karnataka otherwise they will not consider as SSI units of Karnataka.

WHO GMP/GLP / BIS / ISI / ISO / SSI /FDA Other Approved Certificate where ever applicable for the product.
