Annual charges for routine /need based opertion of Siliguri District hospital 4 nos of bed cum passenger lift at Siliguri District hospital as details given below. (Qty-12 Month x 9 Nos. Lift operator Per month= 108 for 04 nos. lift)

Scope of work and Terms & Conditions: The agency will have to depute attendant cum operator (round the clock) for the 03(Three) shifts i.e. Shift 1 : 03 nos lift operators from 06.00 am to 02.00 pm Shift 2 : 03 nos lift operators from 02.00pm to 10.00 pm Shift 3 : 03 nos lift operators from 10.00pm to 06.00 am

The agency shall engage lift attendant with lift operator’s Licence or W.M. Permit 1(b)/II.

The attendant shall sign regularly in time in attendance register kept in the Chamber of the JE / AE and dully verified by the concerned JE / AE.

Except in the case of breakdown, lift operation is must be ensured failing to which proportionate deduction would be imposed by the competent authority from the bill.

Reliever / substitute should be engaged in case of absence of any operater other wise proportionate deduction would be imposed from the bill.

Each of Operator / Attendant should possess a common Cell Phone for each lift and the number should be submitted to the concerned JE / AE.

Operator / Attendant should be capable to rescue the passenger inside the lift in case of lift got stuck in between floors accidentally and inform immediately to the concerned JE / AE.

The contractor shall be responsible for any damage / mischief etc. done by his staffs and the contractor have to compensate the said loss ( excepting any type of accident / theft ).

It will be treated as absent if no operator / attendant found at site.

The agency should pay at least the minimum wages including EPF, ESI & Bonus to workers engaged as per prevailing State Govt. Law. It is desirable that age of workers engage by the agency should neither below twenty years nor exceed sixty years.

The Executive Engineer reserves the right to terminate the Contract at any time with One month’s notice.

The agency must be provide the uniform to the maintenance staff.

The agency must be provide a common mobile no to the staff at site with whatsapp facility.

If any kind of incapability & negligence are noticed in observing all the said terms & conditions against the acceptance/agreement, then necessary deduction in payment will be made in pro-rata basis as decided by the Engineer-In-Charge. In that case the above-said contract may be terminated without issuing any notice and stringent & punishable action may be taken against the company.
