SELF DECLARATION :-The Agency shall give Self Declaration in Rs.200/- Stamp Paper attested by the Government Notary as per the ANNEXURE-II.

GOVERNMENT OF KARNATAKA DEPARTMENT OF LABOUR REGISTRATION CERTIFICATE OF ESTABLISHMENT FORM-C (See Rule 4):-The Agency should have valid Registration Certificate of Establishment issued under Karnataka Shops and Commercial Establishment act of 1961, according to Government of Karnataka, Department of Labour or its equivalent and should have registered with minimum 100 (One Hundred)Labours engaged should be uploaded in original/self attested.

PERFORMANCE CONTRACT STATEMENT & EXPERIENCE CERTIFICATE With WORK ORDERS:-The Agency should have Experience of provided man power to maintenance of Cleaning & Sanitation Services at Government Hospitals/Quasi Government Hospitals/Government Autonomous Medical Institutions/ Any Government Institutions/Private Medical Colleges & Hospitals/Corporate Sectors and should have provided minimum 100 Labours per year for last 02 financial years of 2023-24 and 2024-25 the details as per the Performance Contracts Statement as per the ANNEXURE-III along with EXPERIENCE CERTIFICATES as per ANNEXURE-IV or its equivalent or as per the Principal Employers Format that consisting of required information and its related WORK ORDERS should be uploaded in original/self attested.

PAN CARD:-The Agency or Tenderer should have PAN CARD and should be uploaded in original/self attested.

GST REG 06 CERTIFICATE:-The Agency should have GST REG-06 registration certificate and should be uploaded in original /self attested

GST RETURNS STATEMENT:-GSTR Returns Statement as per Annexure-IX or its equivalent for the last Two Financial years 2023-24 and 2024-25 should be certified by the Commissioner of Commercial Taxes GST or competent authority or Chartered Accountant with UDIN Number in original should be uploaded.

EPF REGISTRATION CERTIFICATE:-The Agency should have valid EPF registration certificate issued by the Office of the Regional Provident Fund Commissioner as per EPF Act and should be uploaded in original.

EPF REMITTANCE STATEMENT WITH CLEARANCE CERTIFICATE:- The Agency should submit the EPF Remittance Statement Annexure-X or its equivalent minimum for Rs.18.00 Lakhs per Annum for having remitted for the Financial year 2023-24 and 2024-25 duly certified by the Office of the Regional Provident Fund Commissioner or competent authority as per EPF Act should be uploaded in original.

ESI REGISTRATION CERTIFICATE:-The Agency must have valid ESI registration certificate issued by the Office of Sub-Regional, ESIC or competent authority as per ESI Act and should be uploaded in original.

ESI REMITTANCE STATEMENT WITH CLEARANCE CERTIFICATE:- The Agency should submit the ESI Remittance Statement Annexure-XI or its equivalent minimum for Rs.4.00 Lakhs per Annum for having remitted for the Financial year 2023-24 and 2024-25 duly certified by the Office of Sub-Regional, ESIC or competent authority should be uploaded in original.

PROFESSIONAL TAX REGISTRATION FORM No.3 CERTIFICATE:- The Agency must have Certificate of Enrolment issued under Karnataka Tax on Professions, Traders, Calling and Employment Act1976 (Form-3) or its equivalent should be uploaded in original.

PROFESSIONAL TAX CLEARANCE CERTIFICATE:- The Agency should submit the Professional Tax clearance certificate as on 31/03/2025 issued by the Commissioner of Commercial Tax Department should be uploaded in original.

ANNUAL TURNOVER CERTIFICATE:-The Agency should submit the Annual Turnover for the last Two financial years i.e. 2023-24 and 2024-25 for having minimum Rs.2.00 Crore turnover per Annum as per ANNEXURE-VIII or provided Man power to maintenance of Cleaning & Sanitation issued & certified by the Chartered Accountant along with UDIN Number should be uploaded in Original.

ITR RETURNS:-Income Tax Returns ITR for the last Two Financial years 2023-24 and 2024-25 in original should be uploaded duly certified by the -Chartered Accountant.

QUALITY CONTROL CERTIFICATE:- The Agency should have valid ISO certificate or its equivalent Quality Control Certificate for Man Power Services and should be uploaded in original.

TENDER OFFER FORM The Tender Offer Form as per the ANNEXURE VII should be uploaded in original

EMD:-The Tenderer should remit Earnest Money in the e-portal and Receipt is to be uploaded in original with seal & Signature
