Proforma statement of the orders up to at least 80% for the primary manufacturer and up to at least 30% for the authorized representative/agent of a manufacturer of the scheduled requirement of the tender for the HPLC Columns in any one of last three years (2 years for Micro and Small Enterprises) (i.e. 2022-23, 2023-24 2024-25) in Section XII and the Performance Statement / Satisfactory Certificates of the same should be submitted.
The bidder must upload either the Firm Registration Certificate, or a certificate of registration issued by an appropriate Government Department, such as the Company Incorporation Certificate or MSME Registration Certificate
The bidder must upload a self-declaration stating that the tenderer has not been blacklisted by any Government Department, Quasi-Government Organization, or Public Sector Undertaking, and is not facing any judicial enquiry and will not supply Refurbished materials
Technical Specifications should be filled (compulsorily with model no, part no and if any other required details), signed, and submitted as per Section - VI
Printed literature and brochure in English of the quoted model including complete details should be uploaded and originals must be submitted to the Member Secretory, TIA, Office of the Chief Scientific Officer, Drugs Testing Laboratory, Bengaluru 560001
Tender bid offer form Section - VIII document duly filled signed and uploaded by the tenderer
If the tenderer is the primary manufacturer, the declaration of primary manufacturer in Section - XV as applicable to be submitted. If the tenderer is other than the primary manufacturer, the Authorization letter from the Primary manufacturer should be in the format provided as per Section XIII to be submitted
Information in Section - XIV Proforma for equipment and quality control employed by the manufacturer to be submitted
The bidder has to upload the copy of the PAN Card
The bidder must upload a copy of the GST Registration Certificate along with the documents of the recent GST return paid challan for the financial year 2024-25
Income tax returns statements for the last three financial years i.e. 2022-23, 2023-24 2024-25 should be uploaded (income tax return acknowledgement copies).
The average annual financial turnover of the tenderer should be minimum of rupees 10 Lakh (it will be 50% with respect to Micro & Small Enterprises) for each of the three financial years i.e. 2022-23, 2023-24 & 2024-25 (last 2 preceding financial years with respect to Micro & Small Enterprises) and certified by a chartered accountant, copy of the annual turnover declaration by a chartered accountant should be submitted
