RATE CONTRACT FOR SUPPLY OF (A) MEDICINES, (B) SURGICAL ITEMS&(C) CONSUMABLES, AGAINST DAY TO DAY LOCAL PURCHASE FOR DIVISIONAL RAILWAY HOSPITAL, SAMBALPUR FOR A PERIOD OF 02(TWO) YEARS FROM THE DATE OF COMMENCEMENT OF THE CONTRACT.RATE OF DISCOUNT IN PERCENTAGE OR AGAINST RUPEE 01 TO BE QUOTED FOR EACH GROUP SEPARATELY BY THE BIDDER.

   SELF DECLARATION : A Self Declaration is to be submitted for ensuring compliance of following conditions: (8.1).Location related to the supply points/hospital at Divisional Railway Hospital, Sambalpur. The licensed drug Retailers/wholesalers should be situated within the distance of 10 kms by road from Divisional Railway Hospital, Sambalpur. [vide RB Circular Sl. 4.(b)(i)] (8.2).Requirement of proper storage, maintenance of stocks, cold chain etc [vide RB Circular Sl. 4.(b)(iii)] (8.3).Services should be available 24x7. (8.4).Supply/delivery of the items shall be done as per the local requirements. (8.5).Items shall be supplied on the same day preferably before the closure of OPD, if the indent has been placed at least 4 hours before closure of OPD timings.
