Supply of different Stationary Items to Para Medical Board

The bidder should have supplied items similar to the type as specified in the Schedule of Requirements and the value of such works should not be less than 80 of the tendered value in any one of the last three years to any state Central Govt Universities, etc However, it will be 40 with respect to Micro & Small Enterprise in any one of the last two years. Regarding this, copies of the supply orders along with the respective satisfactory supply certificates issued by concerned authorities for having supplied similar items in any one of the last three years shall be enclosed. Mere submission of supply order copies is not sufficient and copies of respective satisfactory supply certificates will have to be compulsorily submitted

The Bidder should not have been blacklisted as on the date of bid opening. In this regard, Latest Self-Declaration shall be enclosed which has to be certified by a notary on Rs 100/- Non-Judicial E-Stamp paper and should be compulsorily dated after this tender notification.

Certificate of Registration of Establishment The bidders will have to enclose MSME / Udyam Certificate registered under the Directorate of Industries & Commerce, Govt. of Karnataka. or under any state Govt. / Govt. of India

Samples shall be submitted within the due date as mentioned in the e procurement portal

All the scanned copies of original documents should compulsorily be in colour scanned copies of photocopy xerox will not be accepted for evaluation

The tenderer should furnish the information on all past supplies and satisfactory performance of the items tendered herein, in proforma under Section XI

The rates should be compulsorily quoted for all the items or else the bidder will be rejected. Self-Declaration should be enclosed compulsorily

PAN Card copy and Copies of Income tax Returns filed for the last five years along with Balance sheet and Profit & Loss account shall be enclosed

GST Registration Certificate copy along with copies of last three months GST returns filed shall be enclosed

The average annual turnover of the bidder should not be less than Rs 65,00,000 Rupees Sixty five Lakhs for the last Three Preceding Financial Years (Refer Appendix It will be 50% of the estimated cost of the quantity with respect to Micro and Small Enterprises for the last two preceding financial years The Annual Turnover Statement (Turn Over Certificate) of last 3 years duly Certified by a Registered Chartered Accountant shall be enclosed

The bidder should enclose copies of a minimum of two supply orders for having supplied similar items in a single order of not less than Rs. 20,00,000/- (Rupees Twenty Lakhs) or more.
