NOTES :- If the quoted amount is less than the estimated cost of the work i.e. Rs. 44,56,000/-, then the lowest bidder will have to deposit a FDR in favour of 'GE BHOPAL' within 07 days of opening of financial bid for the difference in estimated cost i.e. Rs. 44,56,000/- and quoted amount. In case the said FDR is not submitted within stipulated period as mentioned above, the contractor shall be banned for 6 months and his bid will be rejected. FDR shall be released only after satisfactorily completion of work.

Outsourcing of Services for running & operation of main electric supply 33 KV receiving sub station at Bairagarh :- including HT distribution and allied system round the clock for 24 hrs in three shifts including sundays & holidays complete all as directed and specified. (a) 33 KV take over witth Main incoming of 33 KV HT power supply for MPKVVCL on point at four pole & two pole structure - 1 Set. (b) HT 33 KV vacuum circuit breaker with CT/PT in incomming and outgoing with remote control pannel - 08 Nos. (c) GOD's and DO fuse assembly 33 KV at MRS and sub stn in area with repair of blown out HT fuses and DO`s - 32 Nos. (d) HT 33 KV Bas bar (33 KV grid) with complete incomming and outgoing network. (e) Battery bank complete, checking of internal wiring, cleaning of terminal, top up battery cells with maintaining the water level of distiled water in batteries. (f) Oiling, greasing of all mechanical components. (g) Cleaning of complete grid area and surrounding area within the fencing of MRS area by cutting removing seasonal grass, bushes , etc and keep power house as in a presentable condition all the time complete as directed by EIC. Notes:- (i) The contractor shall provide all facilities to the workmen to facilitate them in conveyance/ safety. (ii) Minimum wages shall be as applicable for skilled workers under scheduled employment of [Employment on the construction or maintenance of roads or in building operations] and shall be maximum of the minimum wages published by the Central Government under the schedule and the minimum wages published by the Madhya Pradesh Government under the schedule. (iii) The contractor shall submit all the proofs for payment of minimum wages, EPF, ESIC, bonus etc. as per the statutory requirement for claiming any interim payment otherwise they shall submit the acts and the statutory provisions along with the proofs, the provisions, the affidavits etc. which shall have to prove any exemption of the firm and their employ from such statutory requirements. The contractor shall only be paid after satisfying the statutory requirements (iv) The contractor shall pay to his worker in their Aadhaar Linked Bank Account and statement of Bank Account shall be submitted to GE for his verification while claiming interim payment. (v) This contract is for outsourcing of services. The following technical staff to be employed :- Electrician 01 No. in each shift for 03 Shifts (each shift of 08 hrs, shift timings will be as directed by Engineer-in-charge) i.e. 03 Electricians per day

Outsourcing of Services for running & operation of complaint cell :- by deploying Electrician, technically qualitfied with knowledge of English and Hindi language, soft spoken and having knowlege of computer to down load complaints from computer and to physically attend the complaints of "NO LIGHT" including the following duties. (Location for complaint cell will be allocated by Engineer-in-charge). (a) Operator will receive the complaints lodged telephonically or by the person in complaint register or online in computer numbered serially and as per format. (b) Operator will distribute the complaints through complaint sllips to duty electrian, maintain complaint number in docket slip and docket number against complaint as vice-versa including issueing of stores which mention in slips on back side. [Note:-Stores will be issued by the department.] (c) Operator will receive the docket slip after completion/rectification of complaints and will enter the completion report with date & time of completion respectively, details of store used at site. (d) Operator will stack the dockets and paste In order. (e) Operator will maintain and keep update complaint slip & complaint register and put up to JE concerned/Engr in charge daily. (f) Operator will keep liason with AGE E/M office/JE telephonically or personally. (g) Operator in second & third shift (i.e. from 1400hrs to 200 hrs and 200 hrs to 0600 hrs) will attend complaint of NO lights (LT/HT complaint) from pole/DB lodged in the shifts. during these shifts they are to report at 33 KV MRS at 3EME centre. Notes:- (i) The contractor shall provide all facilities to the workmen to facilitate them in conveyance/ safety. (ii) The contractor shall quote the rate for this item under different head as mentioned hereinafter according to the given unit. If any of the quoted rates is lesser than the minimum amount as per the acts and the statutory provisions applicable to that, the offer of the tenderer shall not be considered for acceptance. If the quoted rate for EPF, ESIC, Bonus etc. is lesser than the minimum amount as per the acts and the statutory provisions applicable to that, the offer of the tenderer shall not be considered for acceptance. (iii) Minimum wages shall be as applicable for skilled workers under scheduled employment of [Employment on the construction or maintenance of roads or in building operations] and shall be maximum of the minimum wages published by the Central Government under the schedule and the minimum wages published by the Madhya Pradesh Government under the schedule. (iv) The contractor shall submit all the proofs for payment of minimum wages, EPF, ESIC, bonus etc. as per the statutory requirement for claiming any interim payment otherwise they shall submit the acts and the statutory provisions along with the proofs, the provisions, the affidavits etc. which shall have to prove any exemption of the firm and their employ from such statutory requirements. The contractor shall only be paid after satisfying the statutory requirements and shall be paid on the quoted rates of different heads as mentioned hereinafter except the heads for which the firm shall have been exempted. No payment shall be made for the head for which the firm shall have been exempted. (v) The contractor shall pay to his worker in their Aadhaar Linked Bank Account and statement of Bank Account shall be submitted to GE for his verification while claiming interim payment. (vi) This contract is for outsourcing of services. (vii) Number of days shall be 30 days in a month. Outsoursing of services for Running & Operation of lifts at new MH : (emergency services including preventive & periodical maintenance) of lifts - 4 nos at new MH Bairagrh (Lifts installed in Hospital Bldg in Block 'A', 'B' and 'C; 20 passenger lift (stretcher lift) - 02 Nos, 15 passenger lift - 01 No., 08 passenger lift - 01 No.) including attending emergency services required in lifts round the clock for 24 hrs including sundays & holidays complete all as directed and specified. Outsoursing of services for Running & Operation of central airconditioning plant 250 TR (03 Nos) chilled water type at new MH Bairagarh including 33 Nos AHU's, its control panels, internal and external electrification of installations complete for round the clock for 24 hrs including sundays & holidays and routine maintenance checks as per manufacturers instructions complete all as directed and specified. Notes:- (i) The contractor shall provide all facilities to the workmen to facilitate them in conveyance/ safety. (ii) Minimum wages shall be as applicable for skilled workers under scheduled employment of [Employment on the construction or maintenance of roads or in building operations] and shall be maximum of the minimum wages published by the Central Government under the schedule and the minimum wages published by the Madhya Pradesh Government under the schedule. Refrigeration Mechanic cum AC operator: 01 No. in each shift for 03 Shifts (each shift of 08 hrs, shift timings will be as directed by Engineer-in-charge) i.e. 03 Nos of Refrigeration Mechanic cum AC operator per day.

NOTES (for BOQ serial item No. 1.10 to 4.00 incl sub heads) :- (i) Minimum wages as per Minimum Wages Act 1948 shall be paid to the tradesmen by the contractor. Proper wages paid record shall be maintained and submitted to Engineer-in-charge. Contractor shall supply log book, attendance register, payment register and operator shall be responsible to maintain the log book daily and submit to department duly completed along with attendance register and payment record register at the end of every month. All provisions as laid down in Contract Labour Act 1970 such as payment of minimum fair wages, working conditions, etc shall be complied with by the contractor. (ii) The contractor shall quote the rates for deputing man power and reliever arrangement as per labour commissioners rule requirement of labours as given in BoQ. (iii) The staff employed by the contractor shall be responsible for carrying out cleaning of all panel boards, internal and external portions of power house/lift machine room/AC plant room/Installation in a presentable condition for inspection at all time. If required, the contractor will employ more manpower to carry out the duties assigned above and no extra payment will be made on this account. Rates quoted by him will be deemed to be included, complete all as directed and specified by EIC. (iv) The contractor shall carry out routine check and maintain check sheet daily/ weekly/ monthly all as per manufacturers instructions/SOP and upkeep the installation, rectify normal troubleshootings and will submitt daily updated logsheet at the end of every month in the AGE E/M office. (v) Verification of antecedents of Contractors representatives/ trained personnel/labour deployed at site in connection with execution of work under the contract, as per security requirement of User Unit/installation shall be the responsibility of the contractor and all expenses in connection with verification of antecedents by Police Authority/security Agency shall be borne by the contractor. (vi) Qualification :-ELECTRICIAN/ Refrigeration Mechanic cum AC operator / Lift operator :-The contractor shall provide the services of trained personnel i.e. ITI qualified (Electrician trade/Refrigeration Mechanic cum AC operator / Lift operator as per requirement of Engineer-in-charge) with at least 01 (one) year experience in the relevant field OR matriculate with atleast 03 (five) years experience in the relevant field and requiste certificates in this regard shall be submitted to Engineer-in-charge for their approval before commencement of work and get verification with orignal certificates. (vii) Lift operator should have knowledge of parts of lift, their operation and functions, electric system, ARD system and should know how to rescue the passengers from lift in case of emergency. (viii) Refrigeration Mechanic cum AC operator should have knowledge to operate AC plant, AHU, DG sets / Deisel engine of AC plant and operator is responsible to maintain the diesel upto level in tank, by laisoning with concerned JE and keep the record of POL including maint of log book.

NOTES CONTD.. (for BOQ serial item No. 1.10 to 4.00) (ix) Staff engaged for maintenance/operation of the installation will not leave the installation under any circumstances, if during surprise check by representative of Deptt, maintenance staff found absent then penalty as per PS clause will be imposed on the contractor. (x) Contractor will provide stationary for maintaining Daily log sheet of Power house alongwith attendance sheet. (xi) Proper handing/taking over will be done during shift change and in no case installation will remain unattended. (xii) All tradesmen will be in uniform with name plate/badge as approved by GE, cost of these is deemed to be included in the unit rate quoted by the contractor. (xiii) All tradesmen shall be individually life insured under group insurance scheme or any other scheme for minimum of Rs.1.00 Lakh. Insurance certifcate in this regard shall be submitted to Enginer-in-charge before commencement of work. (xiv) The rate of petty stores such as nuts & bolts, washers, coupling rubber star packing, gland rope, grease, etc are deemed to be included in quoted rates. (xv) The contractor shall provide T & P items to the employers like gland packing, wrenches of different sizes, screw driver set & torch lights, etc. (xvi) Contractor shall provide mobile phone (for keeping liason with the executive staff), rechargeable torch, rain coat (in rainy season), conveyance, etc with the operator/tradesman. The quoted rates are deemed to include for the same. (xvii) Contractor shall provide safety kit like helmet, HT hand gloves, industrial leather shoes, safety belts, set of tools, open wrench set, spanner set, earth rod, screw driver set, elanky set, etc to the opeartors for their use and safety. The quoted rates are deemed to include for the same. In case of non compliane of this requirement a panel recovery of Rs. 3000.00 per tradesman shall be imposed on contractor.
