Copy of the GST tax registration certificate and GSTR-3B challans from January-2025 to January-2026.AND GST Clearance certificate up to Jan-2026 from competent authority

Copy of the Firm prevailing Trade License taken by Local BBMP / TMC/ CMC/ GP/ City Corporation

Copy of the Self declaration in Rs. 100 /- bond paper should be taken in the name of Dean and Director, Haveri Institute Of Medical Sciences, Haveri ie The firm is not in Black listed and any other issues as per annexure-V. Bond Should be taken after tender call dated.

Copy of Annual Turnover Minimum 30.00 Lakhs and above attested by CA for last 3 FINANCIAL years 2022-23,2023-24,2024-25

Income Tax return and Audit Report from Chartered Accountant for Last 3 Years FY 2022-23,2023-24,2024-25 including 3CD,3CB, Profit and loss balance sheet.

Copy of Pan Card

Copy of satisfactorily work completion certificate or work done for the value of 30.00 Lakhs per year regarding the supply of Medical Equipment to the Govt Hospitals/Govt Medical Colleges in Karnataka in the FY 2022-23,2023-24,2024-25(Do not upload Supply/Purchase orders) The suppliers who have supplied to our Institution must submit a satisfactorily work completion certificate obtained from our organization

The Tenderer should upload ISO 9001/2015, ISO 14971:2019 , ISO 24161:2022 and BIS Certificate compulsorily

EMD is Compulsory for all bidders and exemption for MSME with NSIC Certificate holders. Tenderer should upload EMD paid copy

Submit the tender terms and condition copy with seal and signed.

Equipment Name and Company Broacher including specification as per Annexure-6

The tenderer should supply the medical equipment only As per the specifications provided as per annexure-VI

33% Bank Solvency certificate should be produced by the Nationalized banks.(The Solvency certificate must be valid for 90 days from the date of publish of tender). Tenderer Should mention the Kpp Portal tender number(Indent Number) in the solvency certificate

Non-Refundable DD of Rs. 5000 to be drawn in favor of The Director, HIMS, Haveri drawn within the tender period and to be uploaded in the kppp portal. The original DD must be produced at the the time of document verification
