6 Whether the Firm/Company or his blood/Close relations are debarred or blacklisted by any of the Milk Unions, affiliated with the Milkfed Punjab? The details be provided on the separate sheet (in case any of them blacklisted by any Milk Union; the tender is liable to be rejected).
