Copy of the PAN card of the entity, Assessment year of ITRs, Profit & Loss Account and Balance sheets duly certified by the Auditor/Chartered Accountant (2022-23, 2023-24 and 2024-25)

Copy of Audited financial annual turnover statement, by the Auditor / Chartered Accountant having UDIN Number (2022-23, 2023-24 & 2024-25)

Copy of GST registration Certificate and latest GST Return of 6 months from the date of publication of tender

Copy of Letter of Undertaking as per Annexure-B.

Drug Manufacturing License with date of validity.

Valid WHO GMP Certificate wherever applicable

Non-conviction Certificate for the year 2022-23, 2023-24 and 2024-25 from the Drug Licensing Authority of the State in Form 26 E3 of Drugs and Cosmetics Act, 1940.

An affidavit stating that the license of the company has not been cancelled in the past or that it has not been barred from manufacture of the supplied products.

General and Financial information and Declarationduly filled as per Annexure-D.

Test report for every individual Medicines listed in Annexure-F.
