Attested copy of specimen signature of the authorized person who is authorized to sign the bills on behalf of Empaneled Firm or Authorized Entity (AAE/SAE).

Letter of Authorization, authorizing Representative to participate in the RFP and to sign all the documents as per the terms and conditions of this RFP in the letter head of the Original Equipment/Manufacturer / Importer

Relevant and Valid test report of the authorized testing institution: The Applicant who participate in this RFP are required to upload the Commercial test report which contains the name of the item, Model, test report number along with photo of the item of the each of the item being quoted in the e-Procurement Platform. The file name should clearly indicate the name of the item and model along with relevant test report

BIS License: The Applicant who participates in this RFP are required to upload the license for the relevant Equipment

Appendix A - RFP Conditions Acceptance Certificate

Earnest Money Deposit (EMD): Proof of deposit of RFP (EMD amount) in e-procurement portal for an amount as listed in against each item or total EMD amount of all equipment. Hence, MSME (Micro, Small, and Medium Enterprise) certificate, in conjunction with NSIC (National Small Industries Corporation) registration, can be used to obtain an exemption from Earnest Money Deposit (EMD)

Appendix C - Manufacturers Authorization Form duly signed

Annexure I - Technical Proposals 1) Participants Profile, 2) Technical Details & 3) Sales Details for last 3 years

Annexure II - Technical compliance of each item should be given in format. Please note the Applicant to put the required information which is complete in all aspects against each line item

Annexure III - Declaration Certificate of warranty and availability of spares

Annexure IV - Details of Delivery and Service Centers in Karnataka as per the format

Annexure VI - Checklist as per duly completed in all aspects

Annexure VII - Audited Average turnover of Rs.90.00 Lakhs during last three financial years i.e., 2022-23, 2023-24 & 2024-25 with UDI No. on the letterhead of a certified Chartered Accountant

Annexure VIII - IT Returns: The firms should upload/submit IT Returns of past three financial years 2022-23, 2023-24 & 2024-25 (AY 2023-24, 24-25 & 25-26)

Annexure-IX - Affidavit (Declaration)
