Outsourcing of maintenance services for attending day to day complaints of different type of sewerage work such as blockage of WHB, WC, Urinals, Sinks, bottle trap, floor trap, Nahani traps, 'P' or 'S' traps of WCs, gully trap, vertical CI pipe/PVC pipes of any dia including junction holes of any description of various Areas under GE E/M RR Hosp, cleaning of sewage lines, man holes of any size & depth in various sewage shafts within Areas under GE E/M RR Hosp to maintain sewage system fully functional with grass, vegetation, shrubs etc., digging out roots with in sewage shafts completely by crow bars, chisel etc. and cleaning of rain water pipelines laid in any situation for cleaning and removing blockage from entire system and making it fully functional with manual/pressure cleaning pump & flexible rods and other necessary tools and equipment etc. complete all as specified and directed and removal of sludge / mud outside hospital complex, including Sunday by employing following manpower, to entire satisfaction of Engineer-in-Charge with following requirement / condition, complete all as directed.

Notes for Item No 1.00 :- (i)Timing of shift- 0900 to 1700 Hrs. single Shift

(ii) The contractor will have to deploy Minimum 05 Nos. Labourers/Mazdoors every day including Sunday/Holidays to attend day to day sewer complaints.

(iii) The timing of shift(s) may be changed by the Engineer-in-charge as per ground requirements in consultation with user, accordingly, the manpower will be deployed by the contractor. No claim(s) by contractor on this ground will be entertained at any stage. In case of any difference of opinion between Engineer-in-charge and the Contractor in this regards, the decision of the Accepting Officer shall be final, conclusive and binding.

(iv) All the complaint(s) will be addressed and rectified within 02 hrs. from the time of receipt of complaint. If, contractor or his tradesmen fails to attend and rectify the complained within 02 hrs. from the time of receipt of complaint, the compensation/penalty of Rs. 500.00 (Rupees five Hundred only) per complaint per day shall be recovered from the contractor’s dues.

(v) The cost of transportation (to and fro) and other necessary T&P, flexible rods, wires, bamboo sticks and other necessary equipment including mechanical equipment like sewage pumps with all accessories and safety gear shall be arranged / brought by contractor to complete the job and no extra cost shall be paid to the contractor in this effect.

(vi) Any labourers/workmen, at any point of the time during the course of execution of work, at any cost, will not be allowed to manual handling of sewage containing human excreta (i.e. night soil and sludge/sewage containing night soil). All the sludge/mud, containing human night soil, from Manholes, gutters shall be removed by mechanical means by deploying required machinery as and when required. The cost in this effect shall be deemed to be inclusive in the quoted rates only.

(vii) The labourer(s) should be free from the habits of any addiction such as liquor, smoking or other drugs.

(viii) In case, additional manpower other than mentioned here-in-above, will be required for to attend day to day sewer complaints and smooth functioning of sewage system, the contractor shall be solely responsible for providing the same without any extra cost and no extra payment will be made in this affect at any stage.

Outsourcing of maintenance services for attending day to day complaints of different type of joinery work (wooden/steel/aluminium) such as repair/maintenance of iron/aluminium mongery, doors, windows, partitions, pelmet box, door springs, venetian blinds, cupboard, kitchen shelves and easing of doors and windows, replacement of all defective items such as SS wire cloth, door springs, tower bolts, sliding door bolts, knobs, hinges, door stopper, handles, all type of locks and other hardware fitting & fixture, rubber beading, aluminium clips, looking mirror, glass panes, plywood, particle board, MDF board etc. as per site requirements with and all kind of machine works i.e. welding, cutting, bending, drilling, grinding, polishing etc., of various Areas under GE E/M RR Hosp and making connected items/ system fully functional, every day excluding Sunday by employing following manpower as per following requirement, to entire satisfaction of Engineer-in-Charge with following requirement/condition, complete all as directed.

Notes for Item No 2.00 :- (i)Timing of shift- 0900 to 1700 Hrs. single Shift

(ii) The contractor will have to deploy minimum 04 No Carpenter every day excluding Sunday to attend day to day joinery work complaints.

(iii) Any major item/material which are required to be replaced with new one i.e. SS wire cloth, door springs, tower bolts, sliding door bolts, knobs, hinges, door stopper, handles, all type of locks and other hardware fitting & fixture, rubber bidding, aluminium clips, looking mirror, glass panes, plywood, particle board, MDF board etc., shall be issued for "Free for Fixing only", but cost of labour element is deemed to be inclusive in quoted rates.

(iv) The timing of shift(s) may be changed by the Engineer-in-charge as per ground requirements in consultation with user, accordingly, the manpower will be deployed by the contractor for each shift(s). No claim(s) by contractor on this ground will be entertained at any stage. In case of any difference of opinion between Engineer-in-charge and the Contractor in this regards, the decision of the Accepting Officer shall be final, conclusive and binding.

(v) All the complaints received during day time i.e. during regular shift hrs. of a particular day and all the complaints received during odd hrs. of previous day (i.e. after regular shift hrs.), will be addressed and rectified on that particular day only (up to 1800 hrs.). If, contractor or his tradesmen fails to attend and rectify all the complained as mentioned here-in-before, the compensation/ penalty of Rs. 500.00 (Rupees five hundred only) per complaint per day shall be recovered from the contractor’s dues.

(vi) In case, additional manpower other than mentioned here-in-above, will be required to attend all joinery complaints of same day as well as previous day as mentioned here-in-before, the contractor shall be solely responsible for providing the same without any extra cost and no extra payment will be made in this affect at any stage.

(vii) The cost of transportation (to and fro) and other necessary T&P, equipment and safety gear shall be arranged / brought by contractor to complete the job and no extra cost shall be paid to the contractor in this effect.

(viii) All the manpower should not leave the complaint cell till the rectification of all complaints. All the manpower employed by contractor should be free from the habits of any addiction such as liquor, smoking or other drugs.

Outsourcing of maintenance services for attending day to day complaints of different type of water supply & plumbing work such as checking and rectifying water leakage from taps, WCs, flushing cistern, urinals, wash basins, bottle traps etc. and any obstruction to water supply, requiring plumber work such as replacement of washers, check nuts, tightening of fittings, adjustment of float valves and syphon fittings of flushing cistern, cleaning of bottle traps, repair of sensor etc. with and incl replacement of all defective items such as bib tap, mixer, angular stop cock, bottle trap, waste coupling, health faucet, flexible tube hose, grating, waste pipe, flush pipe, syphon KIT, float valve, gate/globe valve, PVC pipes and other sanitary fitting / fixture / spare parts & accessories and pipe fittings, with and all kind of machine works i.e. cutting, treading, bending, drilling, grinding, welding etc., of various Areas under GE E/M RR Hosp, repair/replacement of GI pipe lines joints, valves, cleaning of GI pipe lines of any size & laid in any position in various service shafts / roofs within AHRR complex for rectifying water leakage / blockage and making internal water supply system fully functional, every day including Sunday by employing following manpower, to entire satisfaction of Engineer-in-Charge with following requirement / condition, complete all as directed.

Notes for Item No 3.00 :-(i)Timing of shift- 0900 to 1700 Hrs. single Shift

(ii) The contractor will have to deploy minimum 06 Nos. Plumbers every day including Sunday/Holidays to attend day to day plumbing/internal W/S work complaints.

(iii) Any major items/materials which are required to be replaced with new one i.e. bib tap, wall mixer, basin/sink mixer, pillar cock, angular stop cock, bottle trap, waste coupling, health faucet, flexible tube hose, shower, grating, waste pipe, pipe connection, flushing cistern, flush pipe, syphon KIT, float valve, gate/globe valve, PVC pipes and other sanitary fitting / fixture & accessories and pipe fittings i.e. sockets, elbow, nipple, union, tee, reducer etc., shall be issued for "Free for Fixing only", but cost of labour element is deemed to be inclusive in quoted rates.

(iv)The timing of shift(s) may be changed by the Engineer-in-charge as per ground requirements in consultation with user, accordingly, the manpower will be deployed by the contractor for each shift(s). No claim(s) by contractor on this ground will be entertained at any stage. In case of any difference of opinion between Engineer-in-charge and the Contractor in this regards, the decision of the Accepting Officer shall be final, conclusive and binding.

(v) All the complaint(s) will be addressed and rectified within 02 hrs. from the time of receipt of complaint or any stipulated time given by the JE/ Engineer-in-charge. If, contractor or his tradesmen fails to attend and rectify the complained within 02 hrs. from the time of receipt of complaint or any stipulated time given by the JE/ Engineer-in-charge, the compensation/penalty of Rs. 500.00 (Rupees five Hundred only) per complaint per hour shall be recovered from the contractor’s dues.

(vi) The cost of transportation (to and fro) and other necessary T&P, equipment and safety gear shall be arranged / brought by contractor to complete the job and no extra cost shall be paid to the contractor in this effect.

(vii) All the manpower employed by contractor should be free from the habits of any addiction such as liquor, smoking or other drugs.

(viii) In case, additional manpower other than mentioned here-in-above, will be required for to attend day to day internal water supply complaints and smooth functioning of internal water supply system, the contractor shall be solely responsible for providing the same without any extra cost and no extra payment will be made in this affect at any stage.

Outsourcing of maintenance services for attending day to day complaints of different type of Masonry work such as plaster work (up to 5.0 sqmtr at one place), repair/replacement of broken tiles fixed in any position (up to 2.0 sqmtr at one place), repair of brick work (up to 0.5 cum at one place), repair of PCC/hard stand (up to 0.25 cum at one place), repair of manholes, valve pits, pointing of roofs, repairs to jambs of doors, windows, coping and benching of chajjas etc., of various Areas under GE E/M RR Hosp, every day excluding Sunday by employing following manpower, to entire satisfaction of Engineer-in-Charge with following requirement/condition, complete all as directed.

(a) Mason (Skilled)

(b) Labour (Unskilled)

Notes for Item No 4.00 :-(i)Timing of shift- 0900 to 1700 Hrs. single Shift

(ii) The contractor will have to deploy minimum 03 No Mason every day excluding Sunday to attend day to day Masonry work complaints.

(iii) All the bldg. materials which are required to carry out necessary repair work(s) shall be issued to contractor or his tradesmen within AHRR complex, but cost of labour element is deemed to be inclusive in quoted rates. The cost of transportation / handling of bldg. material from storage place to repair work site is deemed to be inclusive in quoted rates. No claim(s) by contractor in this effect will be entertained at any stage.

(iv)The timing of shift(s) will be decided by the Engineer-in-charge as per ground requirements in consultation with user, accordingly, the manpower will be deployed by the contractor for each shift(s). No claim(s) by contractor on this ground will be entertained at any stage. In case of any difference of opinion between Engineer-in-charge and the Contractor in this regards, the decision of the Accepting Officer shall be final, conclusive and binding.

(v) All the complaints / repair works received/assign on a particular day will be addressed and completed on that particular day only (up to 1800 hrs.) or any stipulated time given by the Engineer-in-charge. If, contractor or his tradesmen fails to attend and complete all the complaints / repair works on the same day or any stipulated time given by the Engineer-in-charge, the compensation/penalty of Rs. 500.00 (Rupees five Hundred only) per complaint / repair work per day shall be recovered from the contractor’s dues.

(vi) In case, additional manpower other than mentioned here-in-above, will be required to carry out necessary masonry repair work(s) of same day, the contractor shall be solely responsible for providing the same without any extra cost and no extra payment will be made in this affect at any stage.

(vii) The cost of transportation (to and fro) and other necessary T&P, equipment and safety gear shall be arranged / brought by contractor to complete the job and no extra cost shall be paid to the contractor in this effect.

(viii)All the manpower should not leave the complaint cell till the rectification of all complaints. All the manpower employed by contractor should be free from the habits of any addiction such as liquor, smoking or other drugs.

Outsourcing of maintenance services for attending day to day complaints of internal electrification/LT supply Such as checking of existing electric items/ circuit for continuous electrical supply or detection of nature of defect/fault, carrying out minor repairs as required to make the items/circuits functional or to restore electric supply i.e. tightening of loose connections, insulating the naked wire, opening of Bakelite sheet/cover plates/DBs etc. and refixng the same after necessary repairs, cutting of Bakelite sheet for fixing of accessories & making all electric connection etc., replacing defective items i.e. switch, socket, fan regulator, MCB, MCCB, ballast, tube rods, casing & capping, conduits, PVC boxs, wires, light luminaries, Bus bar trunking Switchgear Distribution Panel etc. of any type & in any position and other electrical accessories etc., Area under GE E/M RR Hosp, by employing 07 Electrician/ Skilled Labour every day including Sunday/ Holiday as per following shift to entire satisfaction of Engineer-in-Charge complete all as directed as per following :- (a) 3 Nos. electricians per day (one electrician in each shift of 08 hours) for main Bldg. (P-100) (b) 3 Nos. electricians per day (one electrician in each shift of 08 hours) for CT'S (c) 1 Nos. electricians in 01 shift of 08 hours in every day 24 hours for ADC

Outsourcing of maintenance services for attending Day to Day complaints of internal electrification/LT supply Such as No light, Loose connections, short circuits, checking of existing electric items/ circuit for continuous electrical supply or detection of nature of defect/fault, carrying out minor repairs as required to make the items/circuits functional or to restore electric supply i.e. tightening of loose connections, insulating the naked wire, opening of Bakelite sheet/cover plates/DBs etc. and refixng the same after necessary repairs, cutting of Bakelite sheet for fixing of accessories & making all electric connection etc., replacing defective items i.e. switch, socket, fan regulator, MCB, MCCB, ballast, tube rods, casing & capping, conduits, PVC boxs, wires, light luminaries, Bus bar trunking Switchgear Distribution Panel etc. of any type & in any position and other electrical accessories etc., Area under GE E/M RR Hosp, by employing 08 Electrician/ Skilled Labour every day including Sunday/ Holiday as per following shift to entire satisfaction of Engineer-in-Charge complete all as directed for Area(other than mentioned in item No. 5.00 here-in-before) under GE EM RR Hospital as per following :- (a) 05 Nos. electricians in morning shift of 08 hours in every day 24 hours (b) 02 Nos. electricians in 01 (one) evening shift of 08 hours in every day 24 hours (c) 1 Nos. electricians in 01 (one) Night shift of 08 hours in every day 24 hours

Notes for Item No 5.00 & 6.00 :- (i)The electrician(s) must be qualified/skilled enough to understand the electric supply circuits comprise of various LT control panels, switch gears, change over switches, LT busbar and other connected accessories to carried out necessary repair work and to providing uninterrupted power supply to various Areas under GE E/M RR Hosp. He should be well conversant with the lay out diagram of the electric supply and locations of diversions and switch gear etc. and their operations.

(ii) The timing of shift(s) and location will be decided by the Engineer-in-charge as per ground requirements in consultation with user, accordingly, the manpower will be deployed by the contractor for each shift(s) and location. No claim(s) by contractor on this ground will be entertained at any stage. In case of any difference of opinion between Engineer-in-charge and the Contractor in this regards, the decision of the Accepting Officer shall be final, conclusive and binding.

(iii) All the complaint(s) will be addressed and rectified within a same day after receipt of complaint or any stipulated time given by the JE/Engineer-in-charge. If, contractor or his tradesmen fails to attend and rectify the complaint on the same day or any stipulated time given by the JE/Engineer-in-charge, the compensation/penalty of Rs. 500.00 (Rupees five hundred only) per complaint per day shall be recovered from the contractor’s dues.

(iv) In case, additional manpower other than mentioned here-in-above, will be required for to attend day to day internal electric supply complaints and smooth functioning of internal electrification system, the contractor shall be solely responsible for providing the same without any extra cost and no extra payment will be made in this affect at any stage.

(v) Any major items/materials which are required to be replaced with new one i.e. switch, socket, bell push, lamp holder, ceiling rose, fan regulator, modular accessories, ballast / chock, lamp, Bakelite sheet, MCCB, MCB, casing & capping, plug top, light luminaries, PVC box, conduits etc. shall be issued for "Free for Fixing only", but cost of labour element is deemed to be inclusive in quoted rates.

(vi) The cost of transportation (to and fro) and other necessary T&P (i.e. screw driver set each sizes, line tester, combination and cutting pliers, cutting knife, hand gloves suitable for available voltages, Test lamp with conductor and holder etc.), equipment and safety gear shall be arranged / brought by contractor to complete the job and no extra cost shall be paid to the contractor in this effect.

(vii) The electrician(s) and helper(s) should not leave the complaint cell till the next reliever(s) does not report to complaint cell.

(viii) All the manpower employed by contractor should be free from the habits of any addiction such as liquor, smoking or other drugs.

(ix) If any person found absent from particular shift hours, a penalty of Rs. 1500/- per day has been recovered from the contractors dues.

Note for item No. 1.00 to 6.00: - 1. The work will be carried out in multi-storeyed buildings (up to 08 storeyed). Hence, the rates quoted by the contractor shall be deemed to be inclusive of cost of the all elements concern with T&P, labour and other misc item, requisite for executing the work at such heights. No additional claims/ payments against it shall be entertained at any stage of work at any circumstances. 2. As the above work will be executed in multi-storeyed buildings, a group insurance policy for the people working at such heights should be submitted before starting of the work and rates quoted shall be deemed inclusive of all expenditure incurred in getting insurance for the man power employed for this contract. 3. The contractor shall provide all safety equipment such as safety belts, harness, helmets, gumboot, shoes and all other requisite equipment etc. as required to ensure safety of workmen during execution of work and cost of these items are deemed to be inclusive in quoted rates. 4. The contractor will install and maintain a Biometric Based Attendance Monitoring System (BBAMS) in complaint cell and enroll their employed staff in it. The contractor or his Supervisor will submit the attendance records of his employed staff on daily basis to Engineer-in-charge and GE E/M RR Hosp for their perusal. If contractor or his Supervisor fail to submit the attendance records then the payment of employed staff for that week will not be admissible to him. 5. The rates quoted are deemed to be inclusive of Supervisor for supervision of work and also in rainy/cold/changing natural climate.
