If the tenderer is a MSE, Format of Self - Declaration of MSE Registered with NSIC (Appendix - E) is to be filled and uploaded

GST / GST Registration number and copy of Company Registration Certificate has to be uploaded

Format of Self-Declaration of Documents Furnished in Appendix -A i.e. Appendix - D is to be filled and uploaded

Format of Pre shipment sample undertaking (Appendix - I) is to be filled and uploaded

Appendix - M :Proforma for Performance Statement is to be filled and uploaded

Appendix - L: Format of Undertaking of Criminal Case / Conviction is to be filled and uploaded

The tenderer if a trader has to upload authorization letter issued by Manufacturer / Distributor/ Authorized Agent/ Importer as per the specimen copy at Appendix P1 compulsorily if not the bid will be rejected.

The tenderer, if a manufacturer has to upload Manufacturer declaration letter as at Appendix - P2 compulsorily, if not the bid will be rejected.

a.If the tenderer is Non MSEs, upload last three preceding financial years turnover certificate (i.e., 2022-23, 2023-24 and 2024-25) along with copy of audited profit and loss accounts duly certified by Chartered Accountant & ITR for the years mentioned above as proof in this regard. b. If the tenderer is MSEs, upload last two preceding financial years turnover certificate (i.e.2023-24 and 2024-25) along with copy of audited profit and loss accounts duly certified by Chartered Accountant & ITR for the years mentioned above as proof in this regard.
