GST REGISTRATION CERTIFICATE:-GST Registration Certificate (Form REG-06) of the Tenderer in original is to be uploaded.

GST RETURNS (GSTR-3B:- GST Returns (Form GSTR) for the last six months (April-2025 to September-2025) should be certified by the Commissioner of Commercial Taxes GST or competent authority or Chartered Accountant with UDIN Number in original should be uploaded.

ANNUAL TURN-OVER STATEMENT:- The average annual financial turnover of the tenderer should be 80.00 Lakhs during the last 3 preceeding financial years (2022-23, 2023-24 & 2024-25) and It will be 50% i.e Rs.40.00 Lakhs with respect to Micro & Small Enterprises(MSME) for the last 02 preceeding financial years (2023-24 & 2024-25)Turnover Statement should be certified by Commercial Tax Officer or Charted Accountants to be uploaded as per Annexure-II Otherwise will be considered as Non-Responsive.

MANUFACTURING LICENSE:-A. For Manufacturer The Manufacturing license duly renewed up to date is to be uploaded in original.B. For Authorized Dealers/Authorized Importers:-a). Notarized Manufacturing License, duly renewed up to date along with list of products permitted to manufacture for Principle Manufacturer should be uploaded, If Tenderer is an Authorized Agent/Distributor or Authorized Importer

Manufacturers Authorization Certificate:-The Authorization Certificate from original Manufacturer (OEM) is to be uploaded in original from reputed Manufacturers, if Tenderer is an Principle Authorized Agent/Distributor and Licensed Authorized Importer as per Annexure-IV. Note:-The Sub-Agents/Sub-Distributor is not eligible.

Non Conviction Certificate:Latest Non Conviction Certificate issued not older than 6 months of the Tenderer issued by the Drugs Controller as per Annexure-III is to be uploaded in original

TENDER OFFER FORM-PART I:-Tender Offer Form with an undertaking to abide by the terms and conditions of the tender in the format as per Annexure-V-Part-I, of the Tenderer is to be uploaded in original.

DECLARATION FROM-PART II:-Declaration from the Tenderer in the format as per Annexure-VI-Part-II of the Tenderer is to be uploaded in original.

LIST OF EQUIPMENTS QUOTED UNDER THIS TENDER:- List of Equipments/ Instruments along with Manufacturer/s Name, Country of origin and Model quoted by the tenderer should be clearly furnished separately as per Annexure-I, is to be uploaded in original

Valid Drug license issued by drugs controller for supply of Medical gas.Notarized copy of the Drug License issued under the Drugs & Cosmetics Act 1940 and rules, duly renewed up to date for supply of Medical gas is to be uploaded if Tenderer is an Authorized Agent/Authorized Dealer/Manufacturer.

Valid explosive license from the appropriate authority.Notarized copy of the valid explosive license from the appropriate authority is to be uploaded
