| 2.00Scope of work and terms & condition: (1) The Agency must engage 1 no. Operator (Skilled labour) pet shift per day for 3 shifts in a day throughout the year having sufficient knowledge for operation of the DG set satisfactorily. (2) The operator has to operate 2 nos. 125 KVA & 1no. 82.5 KVA Diesel Generator sets at Chandannagar SD Hospital. (3) One log book must be kept in site and maintained by the agency ensuring filling-up the related fields [capacity of DG set, Date, Current at each phase & phase to phase Voltage, duration of power cut-off, DG Running Hour(Start time, stop time, Run Hour), DG KWH generated(Start KWH, stop KWH, Total KWH) etc.] incl. cases of fault/breakdown, if any, in Log book must be incorporated with signature of operator and countersigned by the concerned duty officers of Hospital, as designated/authorized by the Superintendent, Assistant Engineer & Junior Engineer | ||||||
| 2.01(4) The copy of Log book for each month has to be submitted to the concerned duty officers of Hospital, as designated/authorized by the Superintendent, Chandannagar SD Hospital, Hooghly within 7th day of the following month duly countersigning by the respective officer as stated above and subsequently the copy of acknowledgement/receipt of Log book by the Hospital authority to be submitted to this Division Office through proper channel positively otherwise the payment for related bill will not be entertained from the Division Office. (5) The operational staff of the Agency must check followings for satisfactory running of the DG Set :- (a) Engine and Main alternator incl. necessary. Adjustment. (b) Engine oil level, water level of radiator, clean air cleaner element, drain sediments, all belts, batteries incl. its tapping and charging equipments. | ||||||
| 2.02(6) Speed and frequency of DG Set must be maintained as per I.E. Rules. (7) Diesel and engine oil will be provided by the hospital authority. (8) Fuel consumption record should be maintained in a log book on regular basis and should be duly authenticated by the hospital authority. | ||||||
| 2.03(9) Approx. | 2.05(10) Penalty may be imposed for non-operation of the DG Set in time as well as for absence of the operational staff. The decision of the Executive Engineer will be final in this respect. (11) The agency should be responsible for any damage to the DG set due to mishandling / mal-operation done by his staff and shall be liable for the compensation for the same which would be recovered from the bill as per direction of E.I.C. and the accused staff should be replaced immediately. (10) The contract may be terminated with one month’s notice from either side. (11) Quarterly Bill in quadruplicate copied will be submitted to the J.E. by the Agency. (12) The agency must have their office & Telephone contact with Mobile no. and the same should be intimated to this Department in writing. (13) The agency should must submit P.F and E.S.I’s challan and payment acknowledgement document of the respective labours during submission of bill. | ||||||
| 2.06(14) The agency shall maintain one attendance register at site, to be signed regularly by the staff on duty and the same should be authenticated/verified by the concerned Junior Engineer. (15) The agency will have to immediately replace any or all members of the DG operation staff, who has been reported against. (16) The agency shall provide photo identity cards to all the staff employed, including substitutes. (17) Any shut down or fault should be brought to the notice of EIC immediately in writing and over telephone for further course of action. (18) The testing of each DG Set shall be done by the operator weekly for 15 minutes. Cleaning of battery terminals, panels and DG Sets shall be done on weekly basis. | ||||||
| 2.07Space for accommodation of staff of the agency will be provided by the competent authority. (20) If any kind of incapability & negligence are noticed in observing all the above-said terms & conditions, this Department posses the right to deduct the cost from the bills of the contractor on pro-rate basis as determined by the Engineer –In-Charge. In that case, the above-said contract may be terminated without issuing any notice and Stringent & punishable action may be taken against the contractor. (21) The rates should be quoted both in words and figures inclusive of all taxes on monthly basis. No. quotation with price variation clause will be accepted. Rates offered should be stable for the entire period of maintenance. | ||||||
