Proof of manufacturers annual turnover above Rs 2 crores for last 3 years (Average) (2022-23,2023-24 & 2024-25). Authorized Distributor should also upload their manufacturers annual turnover certificate.

Proof of Dealers annual turnover above Rs. 10 Lakhs per year in the last 3 years (Average) (2022-23,2023-24 & 2024-25)

Valid drug license of the supplier

Format mentioning the name of manufacturer and enclosing WHO/GMP certificate or schedule M certification for each Miscellaneous & Others

No conviction certificate of the original manufacturer should not be older than 12 months from the tender notification date

No conviction certificate of the authorized agent in favour Director, KHPIMS, Gadag issued after the date of tender notification.

Self declaration stating that the firm is not black listed with any Government/Quasi Government Organizations and will not supply refurbished item to be furnished on a Rs. 200 Stamp paper

Self Declaration Form (Product wise) as per the format given in the annexures

Tender processing fee in the form of DD favor of Director GIMS Gadag. of Rs.3000/- Should be submitted at the time of Original document verification evaluation. (if you not Paid Tender processing fee that tenderer will be Considered as Non responsive)
