The manufacturing company should have proof of supply of minimum 15 units of the same model rearing units mentioned in Annexure III in Karnataka (references to be mentioned in Annexure II format and purchase order to be enclosed)

Satisfaction/ performance certificate issued from government departments/PSUs/Universities mentioned in Annexure II from Karnataka state for same model mentioned in Annexure-III shall be enclosed duly signed by the organization

Copy of the certificate of Registration of the firm

PAN Card

Provide three years GST returns filed details for 2022-23, 2023-24 & 2024-25

The bidder should not have been blacklisted by any departments of Government of Karnataka or by its public sector undertakings or by any corporate for any reasons whatsoever including corrupt or fraudulent practices or for unfair trade practices (Annexure VI)
