The tenderer should be a manufacturer who must have manufactured, tested and supplied the equipment (s) similar to the type specified in the ‘Schedule of Requirements’ up to at least xxxx % of the quantity required in any one of the last 3 years. The equipments offered for supply must be of the most recent series models incorporating the latest improvements in design. The models should have been released on or after xxxxx and be in satisfactory operation for xxxxx months as on date of tender opening. Certificates to be provided as per Section XII of Tender Document.

In case of authorized representative/dealer of a manufacturer - The manufacturer furnishes authorization in the prescribed format assuring full guarantee and warranty obligations as per General Conditions of Contract and Special Conditions of Contract. Manufacturers Authorization Form to be provided as per Section XIII of Tender Document.

Details of experience and past performance of the tenderer on equipment offered and on those of similar nature within the past three/five years2 and details of current contracts in hand and other commitments.

A detailed description of the essential technical and performance characteristics of the goods.

A list giving full particulars, including available sources and current prices, of spare parts, special tools, etc., necessary for the proper and continuing functioning of the goods for a period of two years, following commencement of the use of the goods by the Purchaser.

Item-by-item commentary on the Purchaser's Technical Specifications demonstrating substantial responsiveness of the goods and services to those specifications or a statement of deviations and exceptions to the provisions of the Technical Specifications.

Latest 2018-19 onwards Purchase Order copies for having supplied and installed similar Items by the authorized dealer or Manufacturer company to any two semi-Government or government Hospitals or Medical Colleges or Institutions or Private Medical Colleges or Hospital or Institutions or Government Departments should be submitted.

The related Users Certificates regarding the satisfactory performance issued by the Government Hospitals or Medical Colleges or Institutions or Private Medical Colleges or Hospital or Institutions to authorized dealer or Manufacturer company should be submitted.

PAN No, IT returns statement, Balance sheet and Profit loss statement of financial year 2022-23, 2023-24 and 2024-25.

Annual turnover statement issued by CA for the financial year 2022-23, 2023-24 and 2024-25. Bidder should have minimum annual turnover of Rs. 100.00 lakhs in each years.

GST registration certificate and latest GST-R-3B in between Apr-26 to Jun-26

Three years Bank Transaction Abstract obtained from the Bank Authorities by mentioning SB or Current Account number for the Years 2022-23, 2023-24 and 2024-25 as per Anx-I Section XV

Signed and Sealed Bid Document, Name of the Firm with Full Address, contact details, E-mail address etc should be submitted.

EMD Payment Challan through e-portal

Local Authority License or valid Firm registration certificate

Import and Export License, if applicable.

Notarized Self declaration on Rs. 100 Stamp Paper as per Annexure-2 section-XVI Regarding Non-Black listing

Quoted Items List showing Item Name, Make, Model and other details should be mentioned on the bidder letter head.

Manufacturing License for the quoted Equipment, if the Tenderer is a Manufacturer

Authorized Agency Certificate from the Principal Manufacturer, if Tenderer is an Authorized Agent or Distributor or Seller as per Section-XII. MAF copies not mandatory for Nursing Articles items.

Undertaking furnished by the Manufacturer that he shall take-up AMC or CMC of the Equipment at the charges quoted if the Tenderer is an Authorized Dealer or Agent or Seller of the Manufactures as per Annexure-4 Section XVIII.

Technical Specifications Information and Brochure of the equipment in original.

Compliance for the Tender specifications and clarifications for deviations, if any.

Quality control Certificate, if the Tenderer is a Manufacturer As per format in Section-XIII
