Bidder should compulsorily upload the notarized copy of IT Returns, Profit and Loss statement and Balance Sheet for the FY 2022-23, 2023-24, 2024-25

The bidder should give undertaking for the compliance to the bid specification provided and should supply medical equipment only as per the specifications provided.

The bidder should upload the notarized affidavit in Rs 500/- stamp paper stating that 5% Bank Guarantee will be given after the tender is being awarded.

Notarized copy of the self-declaration in Rs 100 stamp paper should be taken in the name of dean and director Belagavi institute of medical sciences Belagavi i.e. the firm is not black listed in any of the institutions and the bond should be taken after the tender call date.

The tender should upload the following documents related to the equipment and the OEM. License to manufacture for sale or distribution of CLASS A & CLASS B MEDICAL DEVICES.CE / EC /FDA Certificate from a notified body. BIS/ISO certificate with Standard mark IS/IEC 80601.ISO EN 13485:2016 Certificate related to the OEM.
