List of quoted items as per SJICR format mentioned in the technical bid
GST registration certificate and returns filed for last six months.
Letter of authority issued by the manufacturer authorizing the tenderer to quote their products for the current tender in original.
Drug license issued by the Drug Controller of the respective state.
Manufacturing license in case of items manufactured in India and Valid import license in Form 10 with Form 41 ( as per rule 122A of Drugs & Cosmetics Act) shall be furnished in case the items are imported from outside the country
No-conviction certificate in respect of the Distributor issued by Drug controller of the respective State/ Union Territory for the current year wherever applicable
