Manning & Operation of main receiving station including all sub-station at Base Hospital with 01 Skilled Electrician round the clock including saturday, sunday & holiday in 03 Shifts to maintain equipments and comprising the following shifts complete all as specified and as directed. (a) VCB/OCB-16 Nos (b) Transformer 1000 KVA - 01 Nos (c) Transformer 630 KVA - 03 Nos (d) Transformer 500 KVA - 04 Nos (e) Transformer 400 KVA - 04 Nos (f) Transformer 200 KVA - 01 Nos

Manning & Operation of DG sets by deploying One FGM in each shift of 08 hrs in 3 shifts round the clock including saturday, sunday & holiday at Main Sub Station and Main Commandant office in Base Hospital to maintain the following equipment and complete all as specified and directed in the following shift:- (a) DG Set 40 KVA - 01 Nos (b) DG Set 320 KVA - 01 Nos NOTE:- Total 03 Nos Technician/FGM as follow shall be deployed by the Contractor in three shifts each of 008 Hrs. (i) 1st Shift from 0600 hrs to 1400 hrs. - 01 FGM (ii) 2nd Shift from 1400 hrs to 2200 hrs. - 01 FGM (iii) 3rd Shift from 2200 hrs to 0600 hrs. - 01 FGM

Manning & Operation of DG sets by deploying One FGM in each shift of 08 hrs in 3 shifts round the clock including saturday, sunday & holiday at Old OT in Base Hospital to maintain the following equipment and complete all as specified and directed in the following shift:- (a) DG Set 310 KVA - 01 Nos (b) DG Set 320 KVA - 01 Nos NOTE:- Total 03 Nos Technician/FGM as follow shall be deployed by the Contractor in three shifts each 008 Hrs (I) 1st Shift from 0600 hrs to 1400 hrs. - 01 FGM (II) 2nd Shift from 1400 hrs to 2200 hrs. - 01 FGM (III) 3rd Shift from 2200 hrs to 0600 hrs. - 01 FGM

Note for item No 01 to 03 : (i) The estimated cost of the work in respect of BOQ Sr item Nos 1 to 3 (as based on minimum wages rates issued vide central/state) is Rs 25,35,00.00. If the quoted amount is found less than the estimated cost of Sr item No 01 to 3 of BOQ and tender of lowest bidder is considered for acceptance, then the lowest bidder will have to deposit FDR in favour of Accepting officer for the difference in estimated cost and Accepted amount in respect of BOQ Sr item No 1 to 3 within 30 days from date of acceptance. In case the said FDR is not submitted within 30 days the amount in question shall be recovered from contractor’s first RAR/Final bill whichever is earlier. However the FDR/recovered amount shall be released to the contractor after satisfactory completion of work. Attention is further invited added that when the amount of said recovery mentioned herein above is less than the amount of performance security as indicated under Clause 02 on page 04 of CA and Clause 12 on page 11 of CA, no FDR is required to be submitted for quoting less rate against BOQ Srl Item Nos 01 to 03. However if the amount of performance security is less than the FDR to be submitted for less quoting rate, the difference of performance security amount and FDR amount(for less quoting rates) shall be deposited by the lowest bidder (whose tender is accepted) in the form of FDR for difference amount over and above performance security within 30 days from the date of Acceptance. (ii) One operator/ technician shall perform only one shift of 08 hours in a day. If the same operator found in other shift a recovery of Rs 500/- per day shall be recovered from the contractor's dues. (iii) Contractor shall submit the nominal roll and qualification of operator within 15 days of acceptance of tender. After that penalty for a sum of Rs 500/- per shift per installation shall be recovered from the contractor's dues. (iv) Contractor shall ensure attendance of all operator in shift wise if any operator found absent from duty a sum of Rs 1000/- per shift shall be recovered from the contractor's dues. Any damage of loss on this account shall bear by the contractor.

(v) All operator shall wear proper uniform, failing which a recovery of Rs 200/- per shift shall be recovered from the contractor's dues. (vi) The payment of contractor's employee will be made through nationalized bank account of individual and the proof of transaction and the contractor shall submit the same with bank account pass book of employees and own Bank account statement within 15 days of acceptance otherwise a recovery will be made a sum of Rs 200/- per shift per employee from the contractor's dues. (vii) Contractor will submit the payment proof i.e. passbook of bank account at the time of submission of RAR. (viii) Contractor will arrange their staff to run the installation in Sunday/Public Holiday etc as required basis by Engr-in-Charge. (ix) Sufficient T&P shall be arranged and kept in installation. (x) Shift shall be of 08 hours starting from 0600 to 1400 hrs, 1400 to 2200 hrs and 2200 to 0600 hrs. (xi) Contractor shall be responsible for watch and ward of installation round the clock 24 hours and will operate the installation in 3rd shift on required basis if any. Any loss of damage to the plant equipment installed and Govt. property in the E/M installation premises shall be made good / born by the Contractor. Rate quoted by the Contractor are deemed to be included on above Schedule.

Rewinding only of exhaust fan with required gauge super enamelled wire including varnishing, insulation, taping complete of 300/450mm sweep complete all as specified and as directed.

Taking down defective self starter from DG sets of capacity 40/63 KVA including repair/replacement of carbon brush, stator, rotor winding, bearings and reassembling the same and reinstalled in DG sets testing complete all as specified and as directed.

Supply, laying & connecting in repairs flat 3 core copper conductor PVC insulated & PVC sheathed cable 1100 Volts grade of size 4 Sqmm 56/0.3 laid in and incl 40 mm dia PVC conduit fixed to wall/ in duct complete all as specified and as directed.

Repair to submersible pumping set capacity 02 HP, 440V ,3 Phase :-Opening of submersible motor and cutting to burnt winding and cleaning to slots by cleaning and repair to damage starter etc, rewinding to submersible motor with suitable size of superior quality copper winding wire insulating materials complete all as specified and as directed. Replacement of thrust bearing – 01 No, Replacement of bush – 02 Nos, Re-assembling and transporting to Pump House, lowering pump and testing complete. Dismantling of submersible pump set all as directed and replacement of the following items-(i) Replacement of neck ring – 05 Nos, Replacement of brass bushes– 05 Nos, Replacement of shaft bush- 02 Nos, Replacement of impeller - 05 Nos.
