Bidder Should have Experiences Similar Items Supply and Commissioning of Skill labs in any Three Reputed or Govt three NMC Recognized Medical Teaching Institutes in Karnataka State in last five years

Certified Copy of GST payment receipt along with GST filing acknowledgement GSTR-3B from january-2024 to February-2025

Bidders should have Average 4 Crore Annual Turnover statement as mentioned in Annexure O - IFT in the three financial years (2021-22,2022-23,2023-24).as per Annexure II is to be uploaded. (IT returns will not be considered).

Copies of Balance Sheet Account for three financial years duly certified by the Chartered Accountant/ Auditor, is to be uploaded(2021-22,2022-23,2023-24).

Copies of Profit and Loss Account for three financial years i duly certified by the Chartered Accountant/ Auditor, is to be uploaded. (2021-22,2022-23,2023-24)

The Certificate of incorporation signed by the Registrar of Companies (ROC) notarized is to be uploaded, in case Proprietor/Partnership Firm Registration Certificate/License. Notarized copy should be uploaded.

Tender Form with an undertaking to abide by the terms and conditions of the tender in the format as per Annexure A is to be uploaded

Affidavit has to be submitted by the bidder as per proforma given in Annexure P

Documentary evidence established in accordance for Service Center in Karnataka with maintenance staff and other details to be uploaded and issued by the manufacturer only. As given in Annexure L

If the Bidder claims as an importer the import license shall be in the tenderers name only and should be uploaded for imported items

Manufacturing License obtained from the following competent authority shall be uploaded-if the bidder is manufacturer original Manufacturing License is to be uploaded and if the bidder is authorized supplier/agent/dealer, Notarized Copy of The Manufacturing License is to be uploaded and it is Mandatory).

Manufacturers Authorization as per proforma given in Annexure D

Equipment and Quality Control Employed by manufacturer as per proforma given in As per format given in Annexure G

Declaration for Performance of Quoted Model by manufacturer as per proforma given in As per format given in Annexure M

Technical Literature of the model quoted should be uploaded. As given in As per format given in Annexure I

Technical compliance statements with quoted model and make should be uploaded as per the Annexure I only, otherwise bid will be rejected.

Standard Certificates &Documents ISI ISO CE FDA Certificate along with Certificate as mentioned in the Technical Specifications must be uploaded in original If Tenderer is a Manufacturer If Tenderer is Authorized Agent Authorized Distributor in original or Notarized copy should be up loaded
