Cement Industry/Logistic Partner of Cement Industry/Transporter Should SubmitSatisfactory Work done CertificateIssued by the ULB in the Name of the Entity With respect to Transportation and disposal of RDF to Cement Industries registered in Centralized Extended Producers Responsibility Portal for Plastic Packaging, from any Urban Local Bodies of Karnataka in any of the Year 2023-24, 2024-25, 2025-26

The bidder should submit a valid Service Agreement in the name of Bidder with the cement Industry registered in Centralized Extended Producers Responsibility Portal for Plastic Packaging. Or If the Bidder is Cement Industry, Bidder should submit Authorization issued by Pollution Control Board in the name of Industry

The bidder should submit a valid vendor code issued in the name of Bidder in the letter head of Cement Industry endorsed by the Plant in-charge (seal, name, phone number is must) with cement Industry Or If the Bidder is Cement Industry, Bidder should submit declaration in company letterhead endorsed by the in-charge (seal, name, phone number, Designation) not below the rank of manager.

Bidder should Submit the valid licenses, Goods Carriage Permitsobtained from Competent Authority for providing logistic services Or If the Bidder is Cement Industry, Bidder should submit valid service agreement with logistic partners or submit valid licenses, Goods Carriage Permits obtained from Competent Authority for providing logistic services in the name of bidder

Bidder should have a Minimum of Two Number of GPS fitted Vehicles (Vehicles must be roadworthy, insured, and comply with BS-VI emission norms) in its Name. Or If the Bidder is Cement Industry having service agreement with multiple Logistic Partner, Should submit list of Logistic Partners and Agreement with Logistic Partner who will be deployed for this assignments along with Minimum of Two Number of GPS fitted Vehicles (Vehicles must be roadworthy, insured, and comply with BS-VI emission norms) in the name of Logistic Partner. All the following documents are mandatory vehicle details to be submitted (minimum two vehicles of 18 cum capacity or more than that suitable for this assignment) (Vehicle must be update with all RTO norms) Vehicles shall not be older than 8 years. 1)RC 2)FC 3)PUC 4)Insurance

Bidder should have engaged vehicle drivers having Valid Driving License issued by competent authority for driving heavy vehicle. Submit Drivers Details (minimum two) 1)DL (copy) 2)Badge(ifapplicable) applicable)

Bidder in its name should have achieved in at least two financial years (2023-24 2024-25 2025-26) a minimum of annual financial turnover as mentioned below for the current assignment and should submit audited statement with UDIN a)45 Lakhs (10 marks) b)90 Lakhs (20 marks)

Bidder Should Submit Unconditional Line of credit Rs 27 lakh from any Nationalized or Scheduled Bank
