Hiring of the Transportation vehicles for the supply of Verka pouch milk and fresh milk products in Mohali, Chandigarh, Panchkula, Zirakpur, Ropar, Nangal, Paunta Sahib/Nahan, Pinjore, Kalka, Baddi, Nalagarh, Solan, Delhi and its other areas of operation.

12 Whether any order of blacklisting/debarring the bidder Company or organization passed by any cooperative Organization, Government concern or any sister organization or any State/Public Sector undertaking within the last three years? If yes, attach copy of the order. If no attach affidavit.

13 Whether the requirements as laid down under the Food Safety and Standards Act, 2006 as well as the repealed Act (Known as PFA) are being followed in the earlier works by having the registration/ License under the Rules and Regulation?

14 Bank Account Number for crediting the bills (Attach proof)

15 Fitness certificate, if not then date of applied for fitness certificate, be mentioned
