Notes for item Nos 1.00 to 10.00 here-in-before :- (i) The details of maintenance to be carried out during daily /fortnightly /monthly /quarterly / half yearly /yearly should be all as per details attached in Appendix 'A' Proforma I to VI. (Particular specifications of contract). Log sheet of the maintenance will be submitted by the contractor on monthly basis duly signed by service engineer and contractor. Format of logsheet will be approved by Engr-in-charge. (ii) The rate quoted is inclusive of the cost of all kind of checking & for earth resistance test. (iii) The rate quoted is inclusive of repairs /replacing of earthing system as reqd on ground. (iv) The rate quoted is inclusive of the cost of components generally changed due to wear and tear and not considered under Sch A part III, such as parts of electronic control panel's power contactor, break shoe liner, cables for inter connections, springs, bearings, bushing, gibs, fuses, nuts and bolts etc. (v) The visit of executives/ experts be intimated well in advance so that periodical maintenance can be supervised, schedule of periodical maintenance (daily, fortnightly, monthly, quarterly, half yearly and annually) be submitted well in advance for approval by Engr-in-charge. (vi) The AMC should cover any/ all parts of lifts if its functions found defective the same should be repaired or replaced by the OEM as per the timeline covered under norms. (vii) The contractor has to carry out the AMC of the lifts through OEM only. (viii) All maintenance should be carried out in presence of concerned JE E&M and OEM Authorised Lift engineer maintenance slip will be submitted accordingly. (ix) The rate quoted is inclusive of annual inspection of lift by PWD authorised lift inspector and the report should be submmited to GE office accordingly. (x) The tenderer is required to submit the "MOU" with OEM within 07 days from the start of the work. If this is not done, a penalty of Rs 200/- per day for each lift will be imposed for a duration of one month. Should the contractor still fail to provide the MOU, the contract will be terminated without any prior notice to the contractor.

Providing Rescue Operator (Lift Technician) for passenger lift of capacity 13 passengers (884 Kgs) of Make : M/s Otis, installed at Type-III building NAD Colony from 2200 hours to 0600 hours in one shift of 08 hours each per day, including Sunday and holidays all as per latest applicable lift rules complete all as specified and as directed by Engr-in-charge.
Notes for item Nos 11.00 to 17.00 here-in-before :- (i) The lift Technician / rescuer (operator) shall be Matriculate with sufficient experience (not less than 05 years). The lift rescuer (operator) should be capable to rectify both mechanical as well as electrical defects for smooth functioning of lifts. It is responsibility of contractor to provide a reliever to the rescuer (operator), so that lift rescuer (operator) is available all time during operation period for uninterrupted functioning of lift. (ii) Man power list with Aadhar linked bank account number, qualification certificate will be submitted prior to commencement of Rescue Operation for necessary sanction from GE. (iii) The contractor/ service providing agency shall be wholly and exclusively responsible for payment of wages of persons engaged by it in compliance with all the statutory obligations under all related legislations as applicable to it from time to time including Minimum Wages Act, ESI Act and the Deptt. MES shall not incur any liability for any expenditure whatsoever on the persons employed by the contractor/ Agency on account of any obligation. The contractor/ service providing agency will be required to provide particulars of Group Insurance of its Employees engaged by him to the GE before processing of RAR payment. (iv) The contractor/ service providing agency will be required to provide two sets of uniform to all operators with name and firm tag.
SCHDULE 'A' PART-III (ESSENTIAL REPAIRS) Material and labour for rewinding of double speed electric motor 3 phase, 50 Hz, 415 Volts comprising of following capacity complete all as specified and as directed by Engr-in-charge. (i) Disconnecting and removing electric motor from installed position loading and unloading transporting to nearest workshop. (ii) Dismantling existing burnt winding from motor, cleaning of motor and rewinding complete with super enamelled copper winding wire of existing gauge including insulation material as required. (iii) Bringing back electric motor duly rewinding including loading and unloading transporting from workshop to site of work including electric connection, commissioning and testing in actual working condition for 8 hrs continuously to check RPM, Ampere, load and overall performance. (iv) Replacement of ball bearing - 02 Nos for each motor as per existing size and make RPM.

Dismantling existing unserviceable VVVF drive and supply, fix, test and commissioning variable voltage variable frequency (VVVF) drive with multiple micro program chips based link each other in network, thermal over load protection etc., of make Yashkawa Japan suitable for ECE / OTIS / Johnson make lift to serve upto G+09 floors complete all as specified and as directed by Engr-in-charge.
Supply and fix in replacement main mother board (Logic board) in main panel board after power supply card and giving command to relay card suitable for Johnson / ECE / Escon lift complete all as specified and as directed by Engr-in-charge.

Supply and fix in repair rechargeable, maintenance free lead acid batteries of 12 Volt, 17 AH for ARD system complete all as specified and as directed by Engr-in-charge.

(i) Disconnecting and dismantling from installed position, loading/ unloading and transporting to nearest workshop.
