Schedule `A' Part I

Note (Applicable for Schedule 'A' Part I Sl Item Nos 1.02 to 1.09) : The bidder has to submit an affidavit duly notarised that the rates quoted are above minimum fare wages plus all statutory expenses. This affidavit has to be submitted along with Cover -1 with DD. Otherwise bid will be rejected at technical evaluation stage and if after opening financial bid rates quoted found lower than minimum fare wages plus statutory expenses financial bid will be considered as non bonafide tender and will not be considered for acceptance. The lowest tender among other eligible bidders will be considered as bonafide tender

Outsourcing for the services of 11 KV Sub stn (MH Sub station) including daily maintenance of HT electrical installations comprising of four pole structures/Two pole structures, GODs, VCBs by employing skilled electrician and operation of Diesel Engine generating set 125 KVA installed at Military Hospital comprising of Engine, Alternator by engine filter general machine for a shift of 08 hrs, total 3 shifts / day

Outsourcing for the services of 11 KV Sub stn (Power House Sub Station) including daily maintenance of HT electrical installations comprising of four pole structures/Two pole structures, GODs, VCBs by employing skilled electrician for a shift of 08 hrs, total 2 shifts / day

NOTES: (i) The electrician / operator will be well coversant in handling HT electrical equipments like power transformers, VCBs, OCBs, remote and control panels and replacement of DO fuse elements etc (ii) Keeping the substation neat and clean and in presentable conditions (iii) Proper upkeep of all records such as breakdown details, shut down scheudle of recordings, trivector metre reading, units consumed, daily log sheet etc (iv) Electrician will report immediately to JE / AGE E/M in case of electrical breakdowns (v) No electrical shut downs will be alowed at his disposal without prior permission of JE / AGE E/M (vi) Each Shift shall be of eight hours. The contractor shall employ one operator / electrician per shift. (vii) Time of start of shift shall be as decided by the Engineer-in-Charge. (viii) The contractor shall submit payment proof in Aadhar link Bank account through account payee cheque / RTGS to operator / electrician at the time of submission of RAR and final bill

(ix) The contractors will quote their rates keeping in view the basic rate, employers contribution two words EPF liability, contribution towards ESIC, contribution towards bonus, overhead and profit, labour welfare cess, as applicable depending upon the manpower requirement. The rates quoted shall be inclusive of all taxes, cess, levies as applicable including GST. (x) The quoted rate against bonus will be considered for payment on actual basis. The amount is payable only after submission of valid and authentic documents in support of having paid the requisite amount of bonus to the workmen, contribution towards EPF & ESIC shall also be payable after submission of valid authentic documents in support of proof of payment as applicable depending upon the manpower requirement. Quotation with Nil or Zero consideration (over head and profit) shall be considered as non bonafide.

The rates Quoted by the contractor includes the following :- (a) T&P required for upkeep and maintenance of area including consumables,sweeping brooms etc will be provided by the contractor without extra cost to the Govt b) Contractor shall be solely responsible for any mishappening / accident and loss to life due to negligence of the workmen engaged in the work. Nothing extra / No compensation shall be admissible on this account (c) Each electrician will report to duty station as per duty roster as decided by Engineer-in-Charge. Log sheet shall be signed by the electrician in each shift daily (d) No electrician will leave without reliver and the installation will not be left unmanned in any case (e) Reliver on account of weekly rests / permitted leave will be responsibility of the contractor without any extra cost to the Govt (f) The electrician empolyed by the contractor shall be a ITI holder with minimum four years of experiance in same nature of work.

(g) The contractor will provide protective clothing, hand gloves,testing rods PVC shoes etc without any extra cost to the Govt. (h) Charging / Maintenance of battery etc all as specified by Engineer-in-Charge (j) The location of operator will be changed as per site requirement if required by the Engineer-in - Charge. (k) If the contractor fails to deploy the operator no payment will be made for the same. However a penalty @ double the rate quoted by the contractor will be recovered from his due payment. (l) The police verification of the Electricians deployed by the contractor shall be submittd to this office before starting the work. (m) The contractor shall provide Bio-metric instrument for attendance of operators at all sub stations and same shall be removed after certified completion of the Works. Payments will be made as per attendance of the operator. (n) First Aid Box with necessary medicine shall be provided by the Contractor

Supply, laying, testing & commissioning of LT 1100 KV XLPE insulated screened PVC bedded, electric power cable (heavy duty) with aluminium conductor 1100 volts grade,circular screened with extruded semi conducting compound in combination with copper tape , inner sheath of PVC tape galvanized steel strip flat , around and overall PVC sheathed, armoured conforming to IS 7098 Part II, 1985 amended upto date, of size 3.5 core 120 Sqmm, ISI marked, laid in trenches or passing through pipes/ducts including taking down old cable of any size all as specified and as directed.

Excavating in trenches n exc 1.5 mtr wide and n exc 1.5m in depth , for foundation ,etc or for shafts, wells , cesspits, main holes etc.n exc 10 sqm on plan and n exc 1.5 mtr depth and getting out in soft/ lose soil etc complete all as specified and directed by Engineer-in-Charge.

Returning, filling in, including spreading , levelling , watering and well ramming in layers not exc 25cm of any soil etc complete all as specified and directed by Engineer-in-Charge

Removing excavated materials(soils) n exc 50m and depositing where directed at a level n exc 1.5m above the starting point etc complete all as specified and directed by Engineer-in-Charge.

Supply, laying and replcement of PCC covers, class LV type-1 flat size 300 x 180 x 40mm etc complete all as specified and directed by Engineer-in-Charge, Make : Topaz/Mehetab Tiles or equivalent as approved by GE.

Supply and spread unwashed dry river sand cushioning and covering the under ground cable in trenches etc complete all as specified and directed by Engineer-in-Charge. Note : Punning thickness shall be measured)

An amount of Rs 8475.00 shall be deducted from BOQ on account of schedule of credit
