SECTION IX Earnest Money Deposit Form

Letter of authority issued by the manufacturer authorizing the tenderer to quote their products for the current tender at SJICR in original. The manufacturer should mention the list of products authorizing the distributor to quote in the authorization letter itself.

Drug License issued by the Drug Controller.

Wholesale license of the bidder, issued by the State Drug Control Department and retention of whole sale license held by the bidder.

Import License and retention of import license for imported Drugs.

Relevant manufacturing license /loan license and retention of manufacturing license loan license

Relevant product permission and retention of product permission for drugs and disposables quoted.

No-conviction certificate in respect of the manufacturer and distributor issued by Drug controller of the respective State/ Union Territory for the current year wherever applicable. Annexure-IX.

GMP certificate of the manufacturer

BIS/CE/FDA/DGHS/DGCA/ISI/ISO Technology certificates are mandatory whichever is applicable, failing which the product will not be considered

MD License wherever applicable.

Performance certificate issued by the Drug Controller

Test/analysis report for various formulations of Drugs carried out as per IP 2022 method

GST Registration certificate and returns filed for last six months has to be uploaded

Sample submission acknowledgment issued by Stores Officer, SJICR, Bengaluru.
