Requiredii) A Demand Draft towards Tender Form Fee for Rupees 20,000/-

Requirediii) Annexure-I Technical tender form shall be filled and signed by the tenderer and upload scanned copy in e- procurement platform and hard copies should have submitted.

Requirediv) Annexure-II particulars of vehicle Own/ hired shall be furnished along with vehicle details like RC, Form-24B, fitness, Insurance, state permits, pollutions and lease agreement of the vehicle.

Requiredv) Annexure-III financial undertaking should be furnished in format prescribed.

Requiredvi) Annexure-IV (affidavit-I) attested by a notary to the effect that the Tenderer has no past or present criminal record.

Requiredvii) Annexure-V (Affidavit-II) attested by a notary to the effect that the Tenderer has/have been blacklisted.

Requiredviii) Annexure-VI (Affidavit-III) stating that the tenderer or his representatives are not having any links with the Rice Millers/Mill Lorry Owners Associations/Food grains wholesale.

Requiredix) Annexure-VII (Affidavit-IV) for stating that they have not declared the same stage-II vehicles for another MLS point within district or the MLS points of neighboring districts for obtaining stage -II contracts

Requiredx) Annexure-VIII (Affidavit-V) for stating that no forgery of signatures and fabricated documents submitted for the purpose of award of contract.

Requiredxi) Annexure-IX for Lorry Owner Associations as indicated in clause no 9(iii) of tender terms and conditions shall be uploaded and furnished along with hard copies.

Requiredxii) Copies of Aadhar, GSTIN & PAN should be uploaded.

Optionali) A Demand Draft towards EMD for Rupees 1,90,000/-.
