2 PREQUALIFICATION INFORMATION

Status of the firm (Enter any of the following options: Manufacturers with a minimum annual average turnover of Rs. 25.00 crores during preceding three years )

a Turnover Details shall be furnished for preceeding continuous 3 years out of four years. Rs. in Crores

b The Tenderer who must have supplied the each tendered item up to at least 80% (item wise cumulative) of the tendered quantity to chassis (heavy passenger/goods vehicles) manufacturers / any STUs in any one of the last three completed financial years

a We are the Manufacturers of Automotive Batteries. The itemwise Production capacity and quantity offered to supply to KSRTC per month is mentioned in the additional format.

b We undertake to hold the quoted price for the period of Tender and to submit 5% Bank Guarantee as Security Deposit

c Kilometer Guarantee : Automotive Battery : For city services & Moffusil Services : “The individual battery should perform a minimum of 24 months warranty from the date of fitment to the vehicles. (Fill 'Agree' or 'Not'). This row is to be filled compulsorily for eligibility

d We undertake to abide by all the terms and conditions of the tender and to supply the entire Ordered quantity as per the Delivery Schedule furnished

e Declaration-I as per the format on Govt. stamp paper of Rs. 20 & above

f Declaration-II as per the format on Govt. stamp paper of Rs. 20 & above (In case the manufacturers/ Principal firm will execute supplies through their Channel Partner/ Authorized Dealer/ Distributor)

g Declaration-III as per the format on Govt. stamp paper of Rs. 20 & above

h Affidavit indicating the production capacity and offered capacity along with the machineries held, duly signed by the notary

i Incorpration of the firm as per the factories act - Certificate issued by the Registrar of companies /SSI Certificate to be enclosed

j Statement showing the value of Plant & Machinery, duly attested by the chartered accountant

k Copy of the Pollution control Board clearence certificate with regard to recycling of Scrap Batteries

5 We are aware of the fact that the Tender submitted by us through electronic mode is an valid offer. Any variation committed / observed compared to original documents and conditional offer regarding the payment terms submitted subsequently will lead to disqualification of our offer and also the Corporation reserves the right to Black list us apart from initiating other legal action.
