SUPPLY OF IV FLUIDS TO OBG DEPARTMENT AND SUPPLY OF SYRINGE TO DISTRICT TEACHING HOSPITAL KIMS KOPPAL

For Manufacturer/ Authorized Agent/Distributor The Tenderer must have Supplied the Drugs Items similar to those specified in the Schedule meeting at least 80% of the requirement and amounting Rs.39.25 Lakhs in any one of the last 03 preceeding financial years (2022-23, 2023-24 &2024-25). However for Micro and Small Enterprises it will be 40% quantity required amounting Rs.19.62 Lakhs in any one of the last 02 years (2023-24 & 2024-25).

ANNUAL TURN-OVER STATEMENT:- The Average annual financial turnover of the tenderer should be Rs.49.06 Lakhs during the last 3 preceeding financial years (2022-23, 2023-24 & 2024-25) for Micro & Small Enterprises (MSME) it will be 50% i.e Rs.24.53 Lakhs for the last 02 preceeding financial years (2023-24 & 2024-25). Turnover Statement should be certified by Commercial Tax Officer or Charted Accountants and uploaded as per Section-IX Otherwise will be considered as Non-Responsive.

PAN CARD:-PAN Card of the Firm/Tenderer is to be uploaded in original.

GST REGISTRATION CERTIFICATE:-GST Registration Certificate (Form REG-06) of the Tenderer in original is to be uploaded.

GST RETURNS: RULE 61 (5):- GST Returns RULE 61 (5) for the last six months (05/2025 to 10/2025) should be certified by the Commissioner of Commercial Taxes GST or a competent authority or Chartered Accountant with UDIN Number in original should be uploaded.

I T Returns: Submitt I T returns for last 3 preceeding financial years (2022-23, 2023-24 & 2024-25)

Notarized copy of the Manufacturing license issued under the Drugs & Cosmetics Act 1940 and rules, duly renewed up to date along with list of products permitted (Items to be quoted) must be uploaded if Tenderer is a Manufacturer and the Tenderer is an Authorized Agent/Distributor they must upload the copy of Manufacturers Manufacturing License of his products quoted completely.b) In case of Manufacturing License has expired or will expire, it is insisted to upload the validity certificate from the licensing authority in respect of their license that it continues to be valid during the period of tender process i.e. after submission for the renewal, till the time the license is actually been renewed, is to be uploaded.c) Attested copy of valid SSI certificate issued by Director of Industries Commerce, Government of Karnataka is be uploaded, by SSI units of Karnataka otherwise they will not treated as SSI units of Karnataka.

WHOLE SALE DRUG LICENSE:- If quoted items are permitted to be manufacturer under Drugs & Cosmetics Act 1940 and Rules, and If the Tenderer is an Authorized Agent/Distributor/Importer should upload Original copy of the Whole Sale license issued under drugs & Cosmetics Act 1940 and rules, duly renewed up to date should be uploaded otherwise will be considered as Non Responsive. If wholesale license is not applicable for the quoted items, declaration should be uploaded without fail otherwise will be considered as Non Responsive.

Non Conviction Certificate is to be original to be uploaded. /Notarized Copy: - Latest Non Conviction Certificate issued not older than 6 months in respect of the Manufacturer/ Authorized Agent/Distributors/Importer issued by the Drugs Controller as per Annexure-III is to be original to be uploaded. and the Tenderer is an Authorized Agent/Distributor should also upload the Notarized copy of Manufacturer s Non-Conviction Certificate of his products quoted, Otherwise will be considered as Non-Responsive.

Valid Revised Schedule M-GMP Certificate/ WHO-GMP/ISI/ISO/BSI certificates applicable issued by the concerned licensing authority functioning under the provisions of D&C Act, 1940 and rules there under, covering the categories/products quoted in Original is to be uploaded as per Annexure- IV if Tenderer is a Manufacturer and the Tenderer is an Authorized Agent/ Distributor should upload the Notarized copy of Manufacturer s Revised Schedule M-GMP Certificate/WHO-GMP certificate of his products quoted, Otherwise will be considered as Non-Responsive

Authorization Certificate is to be original to be uploaded.:-The form of Authorization issued by the Manufacturer to the Authorized Agent/Authorized Distributor, referred in Annexure-V in original, is to be uploaded if Tenderer is an Authorized Agent/Distributors, Otherwise will be considered as Non-Responsive

Tender Offer Form is to be original to be uploaded.:-Tender Offer Form with an undertaking to abide by the terms and conditions of the tender in the format as per Annexure-VI-Part-I, of the Tenderer is to be original to be uploaded., Otherwise will be considered as Non-Responsive

Declaration Form is to be original to be uploaded.:-Declaration Form in the format as per Annexure-VI Part II of the Tenderer is to be original to be uploaded., Otherwise will be considered as Non-Responsive.

EMD PAID DETAILS: Payment details copy should be uploaded. EMD Exemption: Uploading MSME Registration with NSIC Certificate and as per the NSIC condition the EMD will be exempted otherwise it will not be exempted.
