Distributor License duly renewed up to date/license validity certificate from Drug Controller for Drugs & Authority Concerned for Non Drugs. if they are Manufacturer then Manufacturing license along with List of Products permitted issued under the Drugs & Cosmetics Act 1940 and Rules, and Up to date original Renewed License along with list of products permitted, for the quoted Items, is to be uploaded, The bidder must have the valid Manufacturer License for the quoted items. Copy of the Self declaration in Rs. 100 /- bond paper should be taken in the name of District Health & Family welfare officer Haveri ie The firm is not in Black listed and any other issues as per annexure-V. Bond Should be taken after tender call dated. Copy of latest Non Conviction Certificate issued by the Drugs Controller as per Annexure-VI or Upload the Acknowledgment copy and submit the original copy at the time of Original document verification.
