Outsourcing of complaint cell, established at Areas under GE E/M Base Hospital for noting down/registering the complaints of various nature pertaining to B/R BHDC & E/M BHDC maintaining the complaint registers for various location/nature, deployment of the tradesman to attend/rectify the complaints, monitoring the progress of rectification work, taking the feedback from user and taking the completion certificate from users, taking attendance records from Biometric Based Attendance Monitoring System (BBAMS) on weekly basis & put up to concerned Engr-in-Charge and GE E/M Base Hospital for their perusal, submitting of complaint attending records on weekly basis & put up to concerned Engr-in-Charge and GE E/M Base Hospital for their perusal, placing indent for issue of various items and keep them in their safe custody, keeping the record of all items issued for repair work, maintaining the inventories of various items, submitting the records of various item consumed and balance qty on weekly basis & put up to concerned Engr-in-Charge and GE E/M Base Hospital for their perusal or any other task / instruction given by Engr-in-Charge, GE E/M Base Hospital for improvement of complaint addressing / rectification system, every day including Sunday & holiday by deploying manpower, to entire satisfaction of Engr-in-Charge with requirement / condition, complete all as specified and as directed.
Notes for Item No 1.00:-
The contractor shall have to deploy minimum 01 No. Computer Operator every day including Sunday and holiday for efficient running the complaint cell.
The Computer Operator must be qualified/skilled enough to handle/run the complaint cell efficiently. The Computer Operator employed shall be having with minimum Four years experience, who will work as interface between the users, MES staff and contractor’s personnel deployed to carry out said maint services. He shall be capable to handle man & material. The Computer Operator must be qualified Graduate/ Ex-Serviceman with adequate knowledge of computer.
The timing of shift(s) and location shall be decided by the Engr-in-Charge as per ground requirements in consultation with user, accordingly, the manpower shall be deployed by the contractor for each shift(s) and location. No claim(s) by contractor on this ground shall be entertained at any stage. In case of any difference of opinion between Engr-in-Charge and the Contractor in this regards, the decision of the Accepting Officer shall be final, conclusive and binding.
In case, additional manpower other than mentioned here-in-above, is required for smooth functioning of complaint cell, the contractor shall be solely responsible for providing the same without any extra cost and no extra payment will be made on this account at any stage.
The cost of all kind of item / materials i.e. computers, peripherals, printer, telephone sets, handsets, furniture, almirahs, stationary items, Biometric Based Attendance Monitoring System etc shall deemed to be inclusive in the quoted rates only.
The cost of transportation (to and from) and other necessary T&P, equipment and safety gear shall be arranged / brought by contractor to complete the job and no extra cost shall be paid to the contractor on this account.
All the manpower employed by contractor should be free from the habits of any addiction such as liquor, smoking or other drugs.
In case, computer operator is found absent during any shift the penal recovery of Rs. 1000/- per shift shall be made from the contractors payment.
The contractor shall also visit the complaint cell once daily and inform defects/ problems if any in writing to Engr-in-Charge as well as GE E/M Base Hospital.
Outsourcing of maintenance services for attending day to day complaints of different type of complaints pertaining to internal electrification / LT supply such as checking of existing electrical items/circuits for continuous electrical supply or detection of nature of defect/fault, carrying out minor repairs as required to make the items/circuits functional or to restore electric supply i.e. tightening of loose connections, insulating the naked wire, opening of bakelite sheet/cover plates/DBs etc and refixng the same after necessary repairs, cutting of bakelite sheet for fixing of accessories & making all electric connection etc, replacing defective items i.e. switch, socket, fan regulator, MCB, MCCB, ballast, tube rods, casing & caping, conduits, PVC boxs, wires, light luminaries of any type & in any position and other electrical accessories etc, with and incl petty repairs items/materials i.e. screws, washers, fuses, small wires of any size and description upto 0.5 mtrs, thimbles (upto 6 sqmm single core wire) and insulation tapes of Areas under GE E/M Base Hospital every day including Sunday by deploying following manpower as per following shift, to entire satisfaction of Engr-in-Charge with following requirement / condition, complete all as specifed and as directed.
Notes for Item No 2.00:-
The contractor shall have to deploy minimum 11 Nos of Electricians every day including Sunday to attend day to day internal/LT electric supply complaints as directed by Engr-in-Charge.
The electrician(s) must be qualified/skilled enough to understand the electric supply circuits comprise of various LT control panels, switch gears, change over switchs, LT busbar and other connected accessories to carried out necessary repair work and to providing uninterrupted power supply to various wards/OPDs, ICU, OT, CCU, NICU and other connected area. He shall be well conversant with the lay out diagram of the electric supply and locations of diversions and switch gear etc and their operations.
The timing of shift(s) and location shall be decided by the Engr-in-Charge as per ground requirements in consultation with user, accordingly, the manpower shall be deployed by the contractor for each shift(s) and location. No claim(s) by contractor on this ground shall be entertained at any stage. In case of any difference of opinion between Engr-in-Charge and the Contractor in this regards, the decision of the Accepting Officer shall be final, conclusive and binding.
All the complaint(s) shall be addressed and rectified within 30 mins from the time of receipt of complaint or any stipulated time given by the JE/Engr-in-Charge. If, contractor or his tradesmen fails to attend and rectify the complained within 30 mins from the time of receipt of complaint or any stipulated time given by the JE/Engr-in-Charge, the compensation/ penalty of Rs. 50.00 (Rupees Fifty only) per complaint per hr for first 02 hrs and after that Rs. 100.00 (Rupees One Hundred only) per complaint per hr shall be recovered from the contractor’s dues.
In case, additional manpower other than mentioned here-in-above, is required to attend day to day internal electric supply complaints and smooth functioning of internal electrification system, the contractor shall be solely responsible for providing the same without any extra cost and no extra payment shall be made on this account at any stage.
The cost of minor items i.e. screws, washers, fuses, small wires of any size and description upto 0.5 mtrs, thimbles (upto 6 sqmm size wire), blanking plates and insulation tapes etc shall deemed to be inclusive in the quoted rates only.
Any major items/materials which are required to be replaced with new one i.e. switch, socket, bell push, lamp holder, ceiling rose, fan regulator, modular accessories, ballast / chock, lamp, bakelite sheet, MCCB, MCB, casing & caping, plug top, light luminaries, PVC box, conduits etc shall be issued for "Free for Fixing only", but cost of labour element shall deemed to be inclusive in quoted rates.
The cost of transportation (to and from) and other necessary T&P (i.e. screw driver set each sizes, line tester, combination and cutting pliers, cutting knife, hand gloves suitable for available voltages, Test lamp with conductor and holder etc), equipment and safety gear shall be arranged / brought by contractor to complete the job and no extra cost shall be paid to the contractor in this effect.
The electrician(s) and helper(s) shall not leave the complaint cell till the next reliever(s) does not report to complaint cell.
All the manpower employed by contractor shall be free from the habits of any addiction such as liquor, smoking or other drugs.
Outsourcing of maintenance services for attending day to day complaints of different type of external water supply & plumbing work such as checking and rectifying water leakage from taps, gate valves, float valves etc and any obstruction to water supply for PVC tanks, requiring plumber work such as replacement of washers, GI sockets, check nuts, tightening of fittings, adjustment of float valves and GI tube fittings of external water supply system etc with and incl replacement of all defective items such as float valve, gate/globe valve, PVC pipes and other external water supply GI fitting / fixture / spare parts & accessories and CPVC pipe fittings, with and incl petty repairs items/materials i.e. screws, washers, nails, plugs,tefnol tape, safeda, thread (soot), M-seal, checknuts etc and all kind of machine works i.e. cutting, treading, bending, drilling, grinding, welding etc, of Areas under GE E/M Base Hospital repair/replacement of GI pipe lines joints, valves, cleaning of GI pipe lines of any size & laid in any position in various service shafts / roofs within BHDC complex for rectifying water leakage / blockage and making external water supply system fully functional, every day including Sunday by deploying following manpower, to entire satisfaction of Engr-in-Charge with following requirement / condition, complete all as specified and as directed.
Notes for Item No 3.00:-
The contractor shall have to deploy minimum 01 No. of Plumbers every day including Sunday to attend day to day plumbing/external water supply work complaints.
The cost of minor items like nails / hooks / nut-bolts / screws / hexa blades / washers / plugs / grease, safeda, thread (soot), M-seal, checknuts, adhesive, white cement etc shall deemed to be inclusive in the quoted rates.
Any major items/materials which are required to be replaced with new one i.e. washers, GI sockets, check nuts, tightening of fittings, adjustment of float valves and GI tube fittings of external water supply system etc and GI pipe fittings i.e. sockets, elbow, nipple, union, tee, reducer etc, shall be issued for "Free for Fixing only", but cost of labour element shall deemed to be inclusive in quoted rates.
The timing of shift(s) shall be decided by the Engr-in-Charge as per ground requirements in consultation with user, accordingly, the manpower shall be deployed by the contractor for each shift(s). Note for Sch - 'A' (BOQ):- The estimated cost of the work in respect of BOQ Sr item Nos 1.00 to 3.00 (as based on minimum wages rates issued vide central/state) is Rs 40,35,000.00. If the quoted amount is found less than the estimated cost of Sr item Nos 1.00 to 3.00 of BOQ and tender of lowest bidder is considered for acceptance, then the lowest bidder will have to deposit FDR in favour of Accepting officer for the difference in estimated cost and Accepted amount in respect of BOQ Sr item Nos 1.00 to 3.00 within 30 days from date of acceptance. In case the said FDR is not submitted within 30 days the amount in question shall be recovered from contractor’s first RAR/ Final bill whichever is earlier. However the FDR/recovered amount shall be released to the contractor after satisfactory completion of work. 1. Attention is further invited added that when the amount of said recovery mentioned herein above is less than the amount of performance security as indicated under Clause 02 on page 04 of CA and Clause 12 on page 11 of CA, no FDR is required to be submitted for quoting less rate against BOQ Srl Item Nos 01 to 03. However if the amount of performance security is less than the FDR to be submitted for less quoting rate, the difference of performance security amount and FDR amount(for less quoting rates) shall be deposited by the lowest bidder (whose tender is accepted) in the form of FDR for difference amount over and above performance security within 30 days from the date of Acceptance.
2. The work shall be carried out in multi-storeyed buildings (upto 02 storeyed). Hence, the rates quoted by the contractor shall be deemed to be inclusive of cost of the all elements concern with T&P, labour and other misc item, requisite for executing the work at such heights. No additional claims/ payments against it shall be entertained at any stage of work at any circumstances. 3. As the above work shall be executed in multi-storeyed buildings, a group insurance policy for the people working at such heights should be submitted before starting of the work and rates quoted shall be deemed inclusive of all expenditure incurred in getting insurance for the man power employed for this contract. 4. The contractor shall provide all safety equipments such as safety belts, harness, helmets, gumboot, shoes and all other requisite equipments etc as required to ensure safety of workmen during execution of work and cost of these items are deemed to be inclusive in quoted rates.
5. The contractor will install and maintain a Biometric Based Attendance Monitoring System (BBAMS) in complaint cell and enroll their employed staff in it. the contractor or his Supervisor will submit the attendance records of his employed staff on daily basis to Engr-in-Charge and GE E/M Base Hospital for their perusal. If contractor or his Supervisor fail to submit the attendance records then the payment of employed staff for that week will not be admissible to him. 6. Contractor shall be bond to implementation to minimum wages as per gov policy ,EPF and ESIC payment and quoted rate shall deemed to include these factors. 7. Contractor shall made payment to the workers throught cheque in account of individual and submit the details of Cheque and bank statement, EPF & ESIC of each workres before asking of any payment (RAR or Final Bill).
