Authorization letter for the firm to sell particular Drugs and Chemicals, letter from the manufacturer.

IT returns of the firm for the last 3 years (2021-22, 2022-23 and 2023-24).

GST registration Certificate.

GST 3B Returns for the Month of Dec- 2025 Or GST Clearance Certificate.

Experience & Past performance in execution of similar supply for the past 3 years (2021-22, 2022-23 and 2023-24).

WHO/GMP Certification or schedule M Certification.

Proof of manufacturers annual turnover above Rs 30 crores per year in the last 3 years (Average) (2021-22, 2022-23 and 2023-24). Authorized Distributor should also upload their manufacturers annual turnover certificate.

Proof of Dealers annual turnover above Rs. 2 crores per year in the last 3 years (Average) (2021-22, 2022-23 and 2023-24).

Valid drug license of the supplier.

Format mentioning the name of manufacturer and enclosing WHO/GMP certificate or schedule M certification for each Tablets (Drugs & Chemicals).

No conviction certificate of the original manufacturer should not be older than 12 months from the tender notification date.

No conviction certificate of the authorized agent in favour Director, KHPIMS, Gadag issued after the date of tender notification.( should not be older than 12 months from the tender notification date.)

Self declaration stating that the firm is not black listed with any Government/Quasi Government Organizations and will not supply refurbished item to be furnished on a Rs. 200 Stamp paper.

Self Declaration Form (Product wise) as per the format given in the annexures.

List of Items Quoted.

All photo copies should be attested by notary.

Tender processing fee in the form of DD favor of Director KHPIMS Gadag of Rs.3000.00 Should be submitted at the time of Original document verification evaluation.
