Upload GST Registration Certificate

Certificate of Incorporation signed by the Registrar of Companies (ROC) In case of a company/firm or original Proprietorship Certificate in case of a proprietary firm in a valid format

Copy of PAN CARD

ISO 9001 2015and GMP as per Revised Schedule M of Drugs and cosmetics Act 1940, Rules 1945 in a valid format

Original Manufacturing License: along with products and composition permitted as listed in the Licence, issued by concerned authoritiesunder the Drugs & Cosmetics Act 1940 and Rules. In case Manufacturing License is under renewal, the bidder should upload the Validity Certificate issued by the Licensing Authority in respect of their Manufacturing license (along with List of Products permitted) clearly indicating that the manufacturing license continues to be valid at least during the Period of tender

MSME Certificate if applicable

Import license Valid Import License issued by Competent Authority exercising powers under Drugs and Cosmetics Act and rules for imported items. Original/Notarized copy of the same shall be Produced at the time of document verification .A bidder quoting for items under import license shall produce all the applicable documents

Average Annual Turnover Statement for preceding three financial years viz., 2022-23, 2023-24& 2024-25 duly certified by Chartered Accountant along with UDIN-Annexure-I

Details of manufacturing Unit and production capacity duly signed by concerned authorised signatory as per the format enclosed as ANNEXURE II. The capacity should be adequate to supply the tendered quantity within the supply period stipulated in the tender

Market Standing statements for2022-23, 2023-24 & 2024-25issued by Chartered Accountant along with UDIN number as per Annexure III

List of items quoted for by the bidder as per Annexure-IV

No Conviction Certificate for the year ending on March 2025 issued by the concerned Drugs Control Authorities/ Licensing Authorities as per Annexure-V

Undertaking to print/emboss logograms as per the format duly signed by the authorized signatory as per Annexure-VII

Format for submission of samples (in duplicate in company letterhead) in annexure VIII.

Tender Offer cum Declaration Form as per Annexure- IX

Form of Authorization in original as per Annexure X

Notarized affidavit declaration by the Tenderer regarding nonblack listing on a Non Judicial stamp paper of value Rs.200/-by Directorate/Commissionerate, Department of Animal Husbandry & Veterinary Services of any states of India at the time of uploading of the Tender - Annexure- XI. In the event of Tenderer found Black listed at the time of uploading the tender or during the contract period and the information has been concealed by the tenderer, the tenderer gets disqualified and contract gets cancelled with immediate effect and fall clause will be invoked
